Description
Overview of the medicine
SMC-Orsaline is an Oral Rehydration Salt (ORS) formulation designed to replenish fluids and electrolytes lost due to dehydration, primarily caused by diarrhea and vomiting. This 1025 gm pack is a bulk quantity for preparing a large volume of solution.
Uses & Indications
Dosage
Adults
The entire 1025 gm powder is for preparing 50 liters of solution. For adults, drink 200-400 ml of prepared solution after each loose stool or as directed by a physician, typically up to 2-4 liters per day, or according to the degree of dehydration and thirst.
Elderly
Same as adults, or as directed by a physician. Caution in those with pre-existing renal or cardiac conditions.
Renal_impairment
Use with caution and under medical supervision in patients with severe renal impairment due to the risk of electrolyte imbalance (hypernatremia, hyperkalemia).
How to Take
Dissolve the entire 1025 gm powder in 50 liters of clean, boiled and cooled drinking water. Mix well until the powder is completely dissolved. Do not add sugar or any other ingredients. Drink the prepared solution slowly over the day, as per dosage instructions. For children, administer small sips frequently.
Mechanism of Action
ORS works by utilizing the co-transport mechanism of glucose and sodium in the small intestine. Glucose facilitates the absorption of sodium and water from the gut into the bloodstream, thereby replenishing lost fluids and electrolytes and correcting dehydration.
Pharmacokinetics
Onset
Fluid and electrolyte restoration begins rapidly upon ingestion.
Excretion
Excess electrolytes are primarily excreted via the kidneys.
Half life
Not applicable in the traditional sense for ORS; effects are maintained by continued intake as needed.
Absorption
Electrolytes (sodium, potassium, chloride, citrate) and glucose are rapidly absorbed from the gastrointestinal tract.
Metabolism
Glucose is metabolized for energy; electrolytes are absorbed and utilized as per physiological needs.
Side Effects
Contraindications
- Severe dehydration requiring intravenous fluid therapy (ORS should be given once the patient can drink).
- Intestinal obstruction, paralytic ileus.
- Severe vomiting preventing oral intake.
- Anuria (absence of urine production) or severe renal failure.
Drug Interactions
No significant drug interactions
ORS is generally safe and does not have significant interactions with most medications. However, caution should be exercised with medications that affect electrolyte balance, especially in patients with compromised renal function.
Storage
Store in a cool, dry place below 30°C. Protect from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose is rare with proper preparation and administration. However, excessive intake, especially in individuals with impaired renal function, may lead to hypernatremia (high sodium levels) or hyperkalemia (high potassium levels). Symptoms may include increased thirst, weakness, confusion, or convulsions. Management involves discontinuing ORS and correcting electrolyte imbalances under medical supervision.
Pregnancy & Lactation
SMC-Orsaline is safe for use during pregnancy and lactation. Dehydration during these periods can be particularly harmful, and ORS provides an effective and safe way to rehydrate.

The efficacy and safety of Oral Rehydration Salts, based on WHO formulation, are well-established through extensive clinical research and widespread use globally, making specific new trials for standard formulations generally unnecessary.
Lab Monitoring
- Routine lab monitoring is not required for uncomplicated dehydration. In severe cases of dehydration, or in patients with pre-existing renal/cardiac conditions, electrolyte levels (sodium, potassium) and renal function may be monitored.
Doctor Notes
- Emphasize the critical importance of correct preparation: using the specified volume of clean water and not adding other substances.
- Advise patients that ORS treats dehydration but does not stop diarrhea; continue feeding, especially breastfeeding.
- Be cautious when prescribing to patients with severe renal impairment due to the risk of hyperkalemia/hypernatremia.
Patient Guidelines
- Always use clean, boiled, and cooled drinking water for preparation.
- Do not add sugar, salt, or any other ingredients to the ORS solution.
- Discard any unused solution after 24 hours.
- Store the powder in a cool, dry place.
Missed Dose Advice
ORS is taken as needed to treat or prevent dehydration. If you miss a dose or delay administration, simply continue giving it as soon as possible if symptoms (diarrhea, vomiting, dehydration) persist. There is no traditional 'missed dose' concept for ORS.
Driving Precautions
SMC-Orsaline has no known effects on the ability to drive or operate machinery.
Lifestyle Advice
- Practice good hygiene, especially hand washing, to prevent infectious diarrhea.
- Continue breastfeeding infants during diarrhea.
- Seek medical attention if dehydration worsens, or if there are signs of severe dehydration (e.g., lethargy, inability to drink).
