Description
Overview of the medicine
Sodib-75 Injection contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID) used for its analgesic, anti-inflammatory, and antipyretic properties. It is administered by injection for rapid relief of acute pain and inflammation.
Uses & Indications
Dosage
Adults
Usually 75 mg by deep intramuscular injection once daily. In severe cases, a second 75 mg injection may be administered 6-8 hours later. Maximum daily dose is 150 mg for no more than 2 days. Switch to oral or suppository therapy as soon as possible.
Elderly
Similar to adults, but use with caution due to increased risk of adverse effects. Lower effective dose should be considered.
Renal_impairment
Use with caution in patients with mild to moderate renal impairment. Avoid in severe renal impairment. Dose reduction may be necessary.
How to Take
Administer by deep intramuscular injection into the gluteal muscle (upper outer quadrant). Not for intravenous (IV) administration unless specifically formulated and indicated. Ensure aseptic technique.
Mechanism of Action
Diclofenac Sodium inhibits prostaglandin synthesis by blocking both cyclo-oxygenase-1 (COX-1) and cyclo-oxygenase-2 (COX-2) enzymes. Prostaglandins are mediators of inflammation, pain, and fever, so their inhibition leads to the therapeutic effects.
Pharmacokinetics
Onset
Within 15-30 minutes after intramuscular injection.
Excretion
Approximately 60% excreted in urine as metabolites, and about 35% excreted in bile.
Half life
Plasma elimination half-life is approximately 1-2 hours.
Absorption
Rapid and complete absorption after intramuscular administration, reaching peak plasma concentrations within 10-30 minutes.
Metabolism
Extensively metabolized in the liver (hepatic metabolism) via hydroxylation and glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to diclofenac, aspirin, or other NSAIDs
- •Active gastrointestinal bleeding, ulceration, or perforation
- •Severe heart failure (NYHA class II-IV)
- •Third trimester of pregnancy
- •Coronary Artery Bypass Graft (CABG) surgery peri-operative pain
- •Severe hepatic or renal impairment
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs and Corticosteroids
Increased risk of gastrointestinal adverse effects.
Diuretics and ACE inhibitors/ARBs
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, and epigastric pain. Gastrointestinal bleeding can occur. In rare cases, hypertension, acute renal failure, respiratory depression, and coma may occur. Management is symptomatic and supportive. Gastric decontamination may be considered if recent oral ingestion.
Pregnancy & Lactation
Pregnancy Category C (first and second trimester), Category D (third trimester). Avoid in the third trimester due to potential risk of premature closure of the fetal ductus arteriosus. Use with caution in first and second trimesters only if potential benefits outweigh risks. Diclofenac is excreted in breast milk; therefore, it is not recommended during lactation.
