Description
Overview of the medicine
Soma DS is a centrally acting skeletal muscle relaxant used for the short-term relief of discomfort associated with acute, painful musculoskeletal conditions in adults.
Uses & Indications
Dosage
Adults
350 mg orally three times daily and at bedtime. Treatment should be for short duration (up to two or three weeks).
Elderly
Use with caution; lower doses may be considered due to increased sensitivity and potential for adverse effects. Avoid in very elderly if possible.
Renal_impairment
Use with caution; carisoprodol and its metabolites are renally excreted. Dose adjustment may be necessary based on renal function.
How to Take
Administer orally, with or without food.
Mechanism of Action
Carisoprodol is thought to act centrally in the reticular formation and spinal cord to reduce muscle spasm. It also has general sedative properties.
Pharmacokinetics
Onset
Muscle relaxation typically occurs within 30 minutes of administration.
Excretion
Carisoprodol and its metabolites are primarily excreted by the kidneys in urine.
Half life
The elimination half-life of carisoprodol is approximately 8 hours, and for its active metabolite meprobamate, it is approximately 10 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzyme CYP2C19, to form meprobamate (an active metabolite) and other inactive metabolites.
Side Effects
Contraindications
- Acute intermittent porphyria
- Hypersensitivity to carisoprodol, meprobamate, or related compounds
Drug Interactions
CYP2C19 inhibitors (e.g., omeprazole, fluoxetine)
May increase carisoprodol exposure, potentially leading to increased adverse effects.
CYP2C19 inducers (e.g., rifampin, St. John's Wort)
May decrease carisoprodol exposure, potentially reducing efficacy.
CNS depressants (e.g., alcohol, benzodiazepines, opioids, tricyclic antidepressants)
Increased sedation, dizziness, and respiratory depression. Concurrent use should be avoided or used with extreme caution.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include stupor, coma, shock, respiratory depression, and seizures. Management involves supportive care, gastric lavage or activated charcoal if recent ingestion, and monitoring vital signs.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy unless the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, not recommended for use during lactation due to potential for sedation in the infant.
