Description
Overview of the medicine
Somopin 3 mg Tablet is an oral somatostatin analog used for the treatment of acromegaly and certain neuroendocrine tumors. It works by mimicking the natural hormone somatostatin, suppressing the secretion of growth hormone and other peptides.
Uses & Indications
Dosage
Adults
The usual starting dose is 3 mg once daily, taken orally. Dose may be adjusted based on clinical response and IGF-1 levels, up to a maximum of 6 mg per day.
Elderly
No specific dose adjustment is typically required in elderly patients; however, renal function should be monitored.
Renal_impairment
For severe renal impairment (CrCl < 30 mL/min), a reduction in the starting dose to 1.5 mg once daily is recommended. Close monitoring is advised.
How to Take
Take Somopin 3 mg Tablet orally with water, preferably on an empty stomach, at least one hour before a meal or two hours after. Do not crush or chew the tablet.
Mechanism of Action
Somopin selectively binds to somatostatin receptors (SSTs), particularly SST2 and SST5, which are highly expressed in growth hormone-secreting pituitary adenomas and various neuroendocrine tumors. This binding inhibits the release of growth hormone (GH), insulin-like growth factor-1 (IGF-1), and other peptide hormones, leading to symptomatic relief and tumor growth inhibition.
Pharmacokinetics
Onset
Pharmacological effects typically observed within a few hours of administration.
Excretion
Excreted mainly through renal (approximately 60%) and biliary (approximately 40%) routes.
Half life
Elimination half-life ranges from 6 to 8 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 2-4 hours. Bioavailability is approximately 30-40%.
Metabolism
Primarily metabolized in the liver via CYP450 enzymes and peptide hydrolysis.
Side Effects
Contraindications
- Hypersensitivity to Somopin or any of its excipients
- Uncontrolled bradycardia
Drug Interactions
Cyclosporine
May decrease cyclosporine absorption, leading to reduced efficacy; monitor cyclosporine levels.
Beta-blockers
May potentiate bradycardia; monitor heart rate.
Bromocriptine
May increase bromocriptine's AUC; dose adjustments may be needed.
Insulin and Oral Hypoglycemics
May alter glucose regulation; monitor blood glucose and adjust antidiabetic medication.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include severe gastrointestinal disturbances, bradycardia, or hyperglycemia. Treatment should be symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Somopin is excreted in human milk; use with caution in breastfeeding mothers.
