Description
Overview of the medicine
Soricap 25 mg capsule contains Acitretin, a retinoid used for severe forms of psoriasis and other keratinization disorders. It works by regulating cell growth and differentiation.
Uses & Indications
Dosage
Adults
Initial dose 25-50 mg daily, taken as a single dose with food. Maintenance dose usually 25-50 mg daily, adjusted based on response and tolerability. Treatment duration typically 6-9 months.
Elderly
No specific dose adjustment required, but caution is advised due to potential comorbidities and reduced renal/hepatic function.
Renal_impairment
Use with caution; dose reduction may be necessary. Not recommended for severe renal impairment.
How to Take
Take orally, preferably with food or milk, once daily. Swallow the capsule whole; do not chew or crush.
Mechanism of Action
Acitretin is a retinoid that works by normalizing the differentiation and proliferation of epidermal cells. It reduces the excessive growth of skin cells characteristic of psoriasis.
Pharmacokinetics
Onset
Therapeutic effects may take several weeks to manifest.
Excretion
Mainly via bile (feces) and urine.
Half life
Approximately 49 hours for acitretin, but can be significantly longer (up to 120 days) for its active metabolite, etretinate, if formed.
Absorption
Well absorbed orally, with bioavailability increased by food.
Metabolism
Primarily hepatic, mainly by isomerization to its active metabolite, 13-cis-acitretin, and by conjugation with glucuronic acid. May also be esterified to etretinate (a teratogenic metabolite with a very long half-life) if alcohol is consumed.
Side Effects
Contraindications
- •Pregnancy and women of childbearing potential (unless effective contraception is used for at least 1 month before, during, and for 3 years after treatment).
- •Breastfeeding.
- •Severe hepatic or renal impairment.
- •Hypersensitivity to Acitretin or other retinoids.
- •Concomitant use with methotrexate or tetracyclines.
Drug Interactions
Alcohol
Increases the risk of formation of etretinate, a teratogenic metabolite with a very long half-life.
Methotrexate
Increased risk of hepatotoxicity.
Tetracyclines
Increased risk of pseudotumor cerebri (benign intracranial hypertension).
Vitamin A and other retinoids
Increased risk of hypervitaminosis A-like effects.
Storage
Store below 25°C, in a dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Symptoms may include headache, nausea, vomiting, dizziness, and dry skin/mucous membranes. Treatment is supportive; discontinue the drug. No specific antidote.
Pregnancy & Lactation
Contraindicated in pregnancy due to severe teratogenicity (causes severe birth defects). Contraindicated during breastfeeding as it is excreted in breast milk.
