Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach. It works by decreasing the amount of acid the stomach makes.
Uses & Indications
Dosage
Adults
Usually 40 mg once daily, taken before breakfast. For severe conditions, dosage may be increased to 80 mg daily. For H. pylori eradication, 40 mg twice daily in combination with antibiotics.
Elderly
No specific dose adjustment required, but caution is advised in cases of severe hepatic impairment.
Renal_impairment
No dose adjustment necessary.
How to Take
Take the tablet whole with a glass of water, preferably 30-60 minutes before breakfast. Do not chew, crush, or break the tablet.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme (the proton pump) on the secretory surface of the parietal cells in the stomach, thereby inhibiting the final step in gastric acid production.
Pharmacokinetics
Onset
Acid secretion inhibition occurs within 2.5 hours, full effect within 2-3 days of continuous therapy.
Excretion
Mainly excreted renally (approximately 71%) and a smaller portion via biliary excretion (approximately 18%) as metabolites.
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, bioavailability is approximately 77%. Peak plasma concentrations achieved within 2.5 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation
- Hypersensitivity to other substituted benzimidazoles
Drug Interactions
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Atazanavir, Nelfinavir
Reduced absorption due to increased gastric pH.
Ketoconazole, Itraconazole
Reduced absorption of pH-dependent drugs.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Treatment is symptomatic and supportive. Pantoprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk; caution advised during lactation.
