Overview of the medicine
Sparel 50 mg Tablet contains Spironolactone, a potassium-sparing diuretic used to treat conditions such as hypertension, edema, heart failure, and hyperaldosteronism. It works by blocking the action of aldosterone, which helps the body get rid of excess sodium and water while retaining potassium.
Uses & Indications
Dosage
Adults
Initial dose 25-100 mg orally once daily or in divided doses; adjusted according to response. Max dose up to 400 mg/day for severe conditions.
Elderly
Start with lower doses and titrate gradually due to increased risk of hyperkalemia and renal impairment.
Renal_impairment
Use with caution; contra-indicated in severe renal impairment (CrCl <10 mL/min) or anuria. Dosage adjustment required based on creatinine clearance.
How to Take
Take Sparel 50 mg Tablet orally, preferably with food or after meals to enhance absorption and reduce gastrointestinal upset. It can be taken once daily or as directed by the physician.
Mechanism of Action
Spironolactone competitively binds to aldosterone receptors in the cortical collecting duct and distal renal tubule, leading to increased excretion of sodium and water and decreased excretion of potassium, thereby acting as a potassium-spsparing diuretic.

Onset
Diuretic effect begins within 2-3 days; peak effect for hypertension takes 2-4 weeks.
Excretion
Primarily excreted in urine (metabolites) and some through feces.
Half life
Parent drug: 1.3 hours; Active metabolites (canrenone, 7α-thiomethylspironolactone): 12-24 hours.
Absorption
Well absorbed from the gastrointestinal tract; absorption is increased when taken with food.
Metabolism
Extensively and rapidly metabolized in the liver to several active metabolites, primarily canrenone and 7α-thiomethylspironolactone.
Side Effects
Contraindications
- •Anuria (absence of urine production)
- •Acute renal insufficiency
- •Significant renal impairment (CrCl <30 mL/min)
- •Hyperkalemia (high potassium levels)
- •Addison's disease
- •Concomitant use with eplerenone or other potassium-sparing diuretics
Drug Interactions
NSAIDs
May reduce diuretic and antihypertensive effects, increased risk of hyperkalemia and renal dysfunction.
Digoxin
Spironolactone may increase digoxin half-life and serum levels.
Lithium
Reduced renal clearance of lithium, leading to increased lithium toxicity.
ACE Inhibitors/ARBs
Increased risk of hyperkalemia.
Potassium Supplements/Potassium-Rich Foods
Significantly increased risk of hyperkalemia.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, nausea, vomiting, dizziness, and hyperkalemia. Management involves inducing emesis or gastric lavage, and symptomatic and supportive treatment, especially managing hyperkalemia.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the benefit outweighs the potential risk. Excreted in breast milk; advise against use during lactation.
