Overview of the medicine
Spilac-F 40 mg Tablet contains Famotidine, an H2-receptor antagonist used to reduce stomach acid production. It is prescribed for the treatment of peptic ulcers, gastroesophageal reflux disease (GERD), and other conditions where excessive stomach acid is a concern.
Uses & Indications
Dosage
Adults
Duodenal/Gastric Ulcer: 40 mg orally once daily at bedtime for 4-8 weeks, or 20 mg twice daily. GERD: 20-40 mg twice daily for 6-12 weeks. Zollinger-Ellison Syndrome: Initial 20 mg every 6 hours; may be increased up to 160 mg every 6 hours.
Elderly
No specific dosage adjustment needed for elderly unless renal impairment is present. Start with lower dose if needed.
Renal_impairment
Creatinine clearance < 10 mL/min: Reduce dose by 50% or extend dosing interval to 36-48 hours.
How to Take
Take orally with or without food. For duodenal ulcer, gastric ulcer, and GERD, it is often taken once daily at bedtime or twice daily. For Zollinger-Ellison Syndrome, it is usually taken multiple times a day.
Mechanism of Action
Famotidine selectively blocks histamine H2-receptors located on the parietal cells of the stomach. This blockade inhibits histamine-stimulated gastric acid secretion, as well as acid secretion stimulated by gastrin and acetylcholine, leading to a reduction in both gastric acid volume and concentration.

Mainly excreted unchanged in the urine (65-70% of absorbed dose). Renal clearance is approximately 250-450 mL/min.
Half life
2.5-3.5 hours (can be prolonged in renal impairment)
Absorption
Rapidly but incompletely absorbed from the gastrointestinal tract; bioavailability is approximately 40-45%. Peak plasma concentrations are achieved within 1-3 hours.
Metabolism
Undergoes minimal hepatic metabolism (25-30%), primarily to an inactive S-oxide metabolite.

Contraindications

Drug Interactions
Antacids
May decrease absorption of famotidine. Separate administration by 1-2 hours.
Cefpodoxime
Famotidine may decrease Cefpodoxime absorption.
Ketoconazole, Itraconazole
Famotidine may reduce absorption of these drugs due to increased gastric pH. Administer several hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, tachycardia, abdominal pain, and hypotension. Management involves symptomatic and supportive treatment. Gastric lavage or activated charcoal may be considered if recent ingestion. Hemodialysis is not effective in removing famotidine.
Pregnancy & Lactation
Pregnancy: Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Lactation: Famotidine is excreted in breast milk. Caution should be exercised when administered to a nursing woman. Consult a doctor.
