Description
Overview of the medicine
Sq-mycetin 0.5% Eye Drop contains Chloramphenicol, a broad-spectrum antibiotic used to treat bacterial infections of the eye, such as bacterial conjunctivitis, keratitis, and other superficial ocular infections caused by susceptible organisms.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the affected eye(s) every 2-4 hours initially, reducing frequency as the infection subsides. Treatment usually continues for 5-7 days or as directed by a physician.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No dosage adjustment necessary for topical ophthalmic use.
How to Take
Wash hands thoroughly before and after use. Tilt head back, pull down the lower eyelid to form a pouch, and instill the prescribed number of drops into the conjunctival sac. Close the eye gently for a few moments. Avoid touching the dropper tip to the eye or any other surface.
Mechanism of Action
Chloramphenicol acts as a bacteriostatic agent by inhibiting bacterial protein synthesis. It binds reversibly to the 50S ribosomal subunit of susceptible bacteria, preventing peptide bond formation and subsequent protein elongation.
Pharmacokinetics
Onset
Clinical improvement typically seen within 2-3 days of starting treatment.
Excretion
Systemically absorbed metabolites are excreted mainly by the kidneys.
Half life
Not well-defined for topical ophthalmic use; systemic half-life is 1.5-5 hours.
Absorption
Minimal systemic absorption occurs after topical ophthalmic administration. Adequate concentrations are achieved in ocular tissues.
Metabolism
Systemically absorbed Chloramphenicol is primarily metabolized in the liver (glucuronidation).
Side Effects
Contraindications
- Known hypersensitivity to Chloramphenicol or any excipients of the formulation.
- History of bone marrow depression or blood dyscrasias.
Drug Interactions
Other eye drops
If using other ophthalmic medications, wait at least 5 minutes between applications to prevent washout.
Systemic Chloramphenicol
Topical use is generally safe, but theoretically, systemic absorption could interact with drugs like warfarin or phenytoin, though highly unlikely with ophthalmic use.
Storage
Store below 25°C, protected from light. Do not freeze. Keep out of the reach of children.
Overdose
Overdose with topical ophthalmic Chloramphenicol is unlikely due to the small quantity administered. If accidentally ingested, symptomatic and supportive measures should be taken.
Pregnancy & Lactation
Use during pregnancy or lactation only if the potential benefit outweighs the potential risk. Consult a physician before use. Minimal systemic absorption suggests low risk for topical use.
