Description
Overview of the medicine
A powdered oral rehydration salt formulation containing electrolytes and glucose, used to treat and prevent dehydration caused by diarrhea or other conditions leading to fluid loss.
Uses & Indications
Dosage
Adults
Dissolve the entire 1025 gm powder in the specified volume of potable water as per pack instructions (e.g., 50 liters for WHO standard formulation). Drink 200-400 ml after each loose stool or as directed by a healthcare professional, ensuring total daily intake meets hydration needs.
Elderly
Same as adults, with careful monitoring for fluid overload in those with cardiac or renal impairment.
Renal_impairment
Use with caution and under medical supervision, especially in severe renal impairment, due to electrolyte content.
How to Take
Oral. Dissolve the powder completely in the recommended volume of clean drinking water as specified on the pack. Do not boil the solution. Drink slowly.
Mechanism of Action
ORS works by utilizing the sodium-glucose co-transport system in the small intestine. Glucose helps the absorption of sodium and water, thus replenishing lost fluids and electrolytes.
Pharmacokinetics
Onset
Within 30-60 minutes after administration.
Excretion
Excess electrolytes are excreted via kidneys; water is excreted through urine, sweat, and respiration.
Half life
Not applicable for a mixture of electrolytes and glucose; components are utilized/excreted continuously.
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Glucose is metabolized for energy; electrolytes are balanced by renal function.
Side Effects
Contraindications
- Intestinal obstruction
- Paralytic ileus
- Severe vomiting making oral intake impossible
- Severe dehydration requiring intravenous fluids (initially)
- Glucose malabsorption
Drug Interactions
None known
Generally, no significant drug interactions reported with standard ORS solution.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children. Once prepared, store the solution in a refrigerator and use within 24 hours.
Overdose
Over-consumption of ORS is unlikely to cause severe overdose in individuals with normal renal function. Symptoms of excessive intake might include hypernatremia or hyperkalemia, especially in impaired renal function. Management involves discontinuing ORS and correcting electrolyte imbalances if severe.
Pregnancy & Lactation
Safe for use during pregnancy and lactation. It's crucial to maintain hydration during these periods.

WHO Essential Medicine
YesClinical Trials
Extensive global clinical trials have established the efficacy and safety of ORS for treating dehydration due to diarrhea, particularly the WHO low-osmolarity formulation.
Lab Monitoring
- Generally not required for routine use. In severe or prolonged dehydration, or in patients with underlying conditions, monitor serum electrolytes (sodium, potassium) and renal function (creatinine, BUN).
Doctor Notes
- Emphasize proper reconstitution with exact water volume to avoid hyperosmolarity.
- Continue feeding and breastfeeding during ORS therapy.
- Advise seeking immediate medical attention for severe dehydration, persistent vomiting, or bloody stools.
Patient Guidelines
- Always use clean, potable water for mixing.
- Mix the entire contents of the pack in the specified volume of water.
- Do not add extra sugar or salt.
- Drink small, frequent sips rather than large amounts at once.
Missed Dose Advice
Continue with the next scheduled dose as needed. ORS is usually taken as needed to replenish fluids and electrolytes, so there isn't a strict 'missed dose' concept like with regular medications.
Driving Precautions
No known effects on driving ability.
Lifestyle Advice
- Maintain good hygiene to prevent further infections.
- Continue feeding (including breastfeeding) during diarrhea.
- Seek medical advice if dehydration worsens or symptoms persist.
