Stosec 20 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is prescribed for conditions like heartburn, acid reflux, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily before breakfast for 4-8 weeks. For severe cases or Zollinger-Ellison Syndrome, dosage may be increased up to 40 mg or more, sometimes twice daily.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule orally, preferably in the morning before breakfast. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase proton pump located in the gastric parietal cells. This action effectively blocks the final step in acid secretion, leading to a significant reduction in stomach acid production.

Onset
Acid suppression begins within 1 hour, maximal effect within 2 hours.
Excretion
Approximately 77% of the dose is excreted in the urine as metabolites, and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 0.5-1 hour, but the duration of anti-secretory effect is much longer.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5-3.5 hours. Bioavailability is about 30-40% but increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Omeprazole, substituted benzimidazoles, or any component of the formulation.
- Co-administration with Nelfinavir (an antiviral drug).

Warfarin
Increased INR and bleeding risk.
Clopidogrel
Reduced antiplatelet effect of clopidogrel (avoid concomitant use).
Methotrexate
Increased plasma levels of methotrexate (monitor closely).
