Description
Overview of the medicine
Gliclazide is a sulfonylurea antidiabetic agent used in the management of non-insulin-dependent diabetes (Type 2) in adults, when diet, physical exercise and weight loss alone are not sufficient. It stimulates insulin secretion from pancreatic beta cells.
Uses & Indications
Dosage
Adults
Initial dose is typically 40-80 mg daily, taken before meals. Dose can be adjusted up to a maximum of 320 mg/day (for standard release). For MR formulations, 30-120 mg once daily, preferably with breakfast.
Elderly
Start with a lower dose and titrate carefully, monitoring blood glucose closely.
Renal_impairment
In mild to moderate renal impairment, dose adjustment may be necessary. Severe impairment is generally contraindicated.
How to Take
Standard release tablets are usually taken before meals. Modified Release (MR) tablets are usually taken once daily with breakfast.
Mechanism of Action
Gliclazide binds to sulfonylurea receptors on the pancreatic beta-cells, closing ATP-sensitive potassium channels. This depolarizes the cell, opening voltage-dependent calcium channels, leading to calcium influx and stimulation of insulin release. It also has some extra-pancreatic effects and microvascular benefits.
