Description
Overview of the medicine
Sucralfate is an antiulcer agent that works by forming a protective barrier over the ulcer surface, shielding it from acid, pepsin, and bile salts. It promotes healing and prevents further damage to the stomach and intestinal lining.
Uses & Indications
Dosage
Adults
1 gm four times daily on an empty stomach (1 hour before meals and at bedtime) or 2 gm twice daily. For maintenance, 1 gm twice daily.
Elderly
No specific dose adjustment for elderly, but use with caution due to potential renal impairment.
Renal_impairment
Use with caution; sucralfate contains aluminum which can accumulate in renal impairment. Consider lower doses and monitor serum aluminum.
How to Take
Administer orally, preferably on an empty stomach, 1 hour before meals and at bedtime. Shake the suspension well before use.
Mechanism of Action
Sucralfate selectively binds to proteins at the base of ulcers and erosions, forming a protective gel-like coating that shields the injured mucosa from further damage by gastric acid, pepsin, and bile. It also stimulates prostaglandin synthesis, bicarbonate secretion, and angiogenesis.
Pharmacokinetics
Onset
Within 30 minutes to 1 hour (for coating effect).
Excretion
Excreted largely unchanged in feces. Absorbed portion excreted in urine.
Half life
Systemic half-life is short due to minimal absorption.
Absorption
Minimally absorbed (less than 5%).
Metabolism
Not significantly metabolized.
Side Effects
Contraindications
- Known hypersensitivity to sucralfate or any component of the formulation
Drug Interactions
Antacids
Should not be taken within 30 minutes before or after sucralfate as they may interfere with sucralfate's binding.
Proton Pump Inhibitors (e.g., Omeprazole)
Similar to H2 blockers, may reduce efficacy if taken simultaneously due to pH elevation. Administer at different times.
H2 Blockers (e.g., Cimetidine, Ranitidine)
May reduce sucralfate's efficacy if taken simultaneously due to pH elevation. Administer at different times.
Tetracycline, Digoxin, Phenytoin, Quinidine, Theophylline, Warfarin
Sucralfate may reduce the absorption of these drugs. Administer these drugs at least 2 hours before or after sucralfate.
Storage
Store at controlled room temperature (20-25°C), protect from freezing. Keep out of reach of children.
Overdose
Overdose with sucralfate is rare due to its minimal systemic absorption and low toxicity. Symptoms are usually mild and may include indigestion, nausea, and vomiting. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women. Use with caution during pregnancy if clearly needed. It is unknown whether sucralfate is excreted in human milk, use with caution in nursing mothers.

Clinical Trials
Numerous clinical trials have established sucralfate's efficacy in treating and preventing peptic ulcers, demonstrating its ability to accelerate healing and reduce recurrence rates. Ongoing research may explore its role in other GI conditions.
Lab Monitoring
- Serum aluminum levels (especially in patients with chronic renal failure)
- Monitor for electrolyte imbalances with prolonged use (e.g., hypophosphatemia, though rare)
Doctor Notes
- Advise patients to avoid taking other medications within 2 hours of sucralfate.
- Counsel patients on the importance of taking sucralfate on an empty stomach for optimal efficacy.
- Monitor renal function and serum aluminum levels in patients with impaired kidney function on long-term sucralfate therapy.
Patient Guidelines
- Take this medication exactly as prescribed by your doctor.
- Shake the suspension well before each use.
- Take on an empty stomach, typically 1 hour before meals and at bedtime.
- Do not take antacids or other medications within 2 hours of taking sucralfate unless directed by your doctor.

Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
