Description
Overview of the medicine
Sudoxim (Cefixime) is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including otitis media, pharyngitis, tonsillitis, bronchitis, urinary tract infections, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
200-400 mg once daily or 200 mg every 12 hours, depending on the severity of infection. Duration usually 7-14 days.
Elderly
Similar to adults, but renal function should be assessed, and dose adjusted if necessary.
Renal_impairment
Dose adjustment required. For creatinine clearance < 60 mL/min, dose should be reduced (e.g., 200 mg once daily).
How to Take
Can be taken orally with or without food. However, taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, essential for bacterial cell wall strength and rigidity. This leads to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved between 2-6 hours after oral administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some is also excreted via bile.
Half life
The average plasma half-life is 3-4 hours, but may be prolonged in patients with renal impairment.
Absorption
Approximately 40-50% is absorbed orally. Food may delay time to peak concentration but does not significantly affect the extent of absorption.
Metabolism
Cefixime is not extensively metabolized in the body. A small portion may be metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin.
Drug Interactions
Warfarin
May increase prothrombin time (INR), requiring closer monitoring and dose adjustment of anticoagulants.
Nifedipine
May increase cefixime levels.
Carbamazepine
May increase carbamazepine plasma concentrations.
Storage
Store at room temperature (below 30°C), protect from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C).
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Excreted in small amounts into breast milk; caution should be exercised when administered to a nursing woman.
