Overview of the medicine
Gliclazide is an oral hypoglycemic agent of the sulfonylurea class, used to manage blood glucose levels in adults with type 2 diabetes mellitus. It stimulates insulin release from pancreatic beta cells.
Uses & Indications
Dosage
Adults
Initially 40-80 mg daily, taken with breakfast. Dosage can be adjusted in increments of 40 mg, usually up to 320 mg daily, divided into two doses if exceeding 160 mg.
Elderly
Similar to adult dosage, but titration should be slow, starting with 40 mg daily.
Renal_impairment
No dose adjustment is generally required for mild to moderate renal impairment, but caution is advised, and close monitoring is recommended. Avoid in severe renal impairment.
How to Take
Orally, preferably with breakfast. For doses exceeding 160 mg, it is usually divided into two doses. Swallow whole with water, do not crush or chew.
Mechanism of Action
Gliclazide primarily works by stimulating insulin secretion from the pancreatic beta cells. It binds to the sulfonylurea receptor (SUR1) on the beta-cell membrane, leading to depolarization, calcium influx, and subsequent insulin release. It may also have extrapancreatic effects.

Primarily via urine as metabolites, with a small amount excreted in feces.
Half life
Approximately 10-12 hours for the 80 mg formulation.

Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to gliclazide, other sulfonylureas, or sulfonamides.

- •Diabetic ketoacidosis.
- •Severe renal or hepatic impairment.
- •Concomitant treatment with miconazole.
- •Pregnancy and lactation.
Drug Interactions
Miconazole
Enhances the hypoglycemic effect, potentially leading to severe hypoglycemia.
Beta-blockers, ACE inhibitors
May potentiate the hypoglycemic effect.
Phenylbutazone, Danazol, Alcohol
May increase the hypoglycemic effect.
Corticosteroids, Diuretics, Estrogens
May reduce the hypoglycemic effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypoglycemia (e.g., sweating, tremor, hunger, confusion). Treatment involves immediate administration of carbohydrates (sugar, sweet drink), followed by careful monitoring. Severe hypoglycemia may require intravenous glucose or glucagon.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Insulin therapy is recommended for diabetic pregnant women.
