Description
Overview of the medicine
Sulfazin 500 mg Tablet is an antibiotic belonging to the sulfonamide class, used to treat various bacterial infections, including urinary tract infections, nocardiosis, and in combination for toxoplasmosis. It works by inhibiting bacterial growth.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g, 2-4 times daily, depending on the infection severity and type. For UTIs: 1 g initially, then 500 mg 4 times daily.
Elderly
Lower doses may be required due to age-related decrease in renal function. Close monitoring for adverse effects is recommended.
Renal_impairment
Dosage adjustment is necessary. For mild to moderate impairment, frequency or dose should be reduced. Contraindicated in severe renal failure.
How to Take
Take orally with a full glass of water, with or without food, to prevent crystalluria. Complete the full prescribed course of treatment even if symptoms improve.
Mechanism of Action
Sulfonamides inhibit bacterial folic acid synthesis by competitively antagonizing para-aminobenzoic acid (PABA), which is essential for bacterial growth and replication. This leads to a bacteriostatic effect.
Pharmacokinetics
Onset
Within 1-2 hours.
Excretion
Primarily excreted via the kidneys, both as unchanged drug and metabolites, through glomerular filtration and tubular secretion.
Half life
Approximately 7-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 2-6 hours after oral administration.
Metabolism
Partially metabolized in the liver, primarily by N-acetylation, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to sulfonamides or any component of the formulation
- Severe hepatic or renal impairment
- Porphyria
- Infants under 2 months of age (due to risk of kernicterus)
- Pregnancy at term and during lactation
Drug Interactions
Warfarin
May enhance the anticoagulant effect, requiring close monitoring of INR and possible dose adjustment.
Phenytoin
Can increase phenytoin plasma levels, leading to toxicity.
Methotrexate
May increase methotrexate toxicity due to competition for plasma protein binding and renal excretion.
Oral Hypoglycemics (Sulfonylureas)
May enhance hypoglycemic effect.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach and sight of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, headache, mental depression, and crystalluria. In severe cases, hematuria, anuria, and toxic epidermal necrolysis may occur. Management involves gastric lavage, forced diuresis (alkaline urine helps), and symptomatic supportive care.
Pregnancy & Lactation
Not recommended during the last trimester of pregnancy due to the risk of kernicterus in the newborn. Use with caution during lactation; consult a doctor as small amounts may pass into breast milk.

YesClinical Trials
Sulfonamides, including Sulfazin, have been extensively studied since the mid-20th century. Clinical trials have demonstrated their efficacy against a broad spectrum of bacterial pathogens, particularly in treating urinary tract and respiratory infections.
Lab Monitoring
- Complete blood count (CBC) should be monitored regularly, especially during prolonged therapy, to detect blood dyscrasias.
- Renal function tests (e.g., serum creatinine, BUN) should be performed periodically.
- Urinalysis for crystalluria and hematuria is advised.
Doctor Notes
- Emphasize adequate hydration (2-3 L/day) to patients to minimize the risk of crystalluria.
- Monitor CBC and renal function, especially in patients on prolonged therapy or with pre-existing renal impairment.
- Advise patients about potential photosensitivity and to avoid excessive sun exposure.
- Be alert for severe skin reactions like SJS/TEN; discontinue immediately at the first sign of rash.
Patient Guidelines
- Drink plenty of fluids (2-3 liters daily) to prevent crystalluria and kidney stone formation.
- Complete the entire course of medication as prescribed by your doctor, even if your symptoms improve.
- Avoid prolonged exposure to sunlight or tanning beds, and use sunscreen, as this medicine can cause photosensitivity.
- Report any severe skin rash, fever, sore throat, or unusual bleeding/bruising immediately to your doctor.
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Sulfazin may cause dizziness, headache, or visual disturbances in some individuals. Patients should be cautious when driving or operating machinery until they know how the medication affects them.
Lifestyle Advice
- Maintain good personal hygiene, especially during urinary tract infections.
- Avoid alcohol consumption during the treatment period, as it may exacerbate side effects.
- Eat a balanced diet to support overall health.
