Description
Overview of the medicine
Sulin 100 mg Tablet contains Nimesulide, a non-steroidal anti-inflammatory drug (NSAID) used for the symptomatic treatment of acute pain and primary dysmenorrhea.
Uses & Indications
Dosage
Adults
100 mg twice daily after meals. Maximum daily dose is 200 mg. Treatment duration should be as short as possible and not exceed 15 days.
Elderly
No specific dose adjustment is usually required, but caution is advised due to potential for decreased renal function.
Renal_impairment
Contraindicated in severe renal impairment. In moderate impairment, dosage should be carefully considered, possibly reducing to 100 mg once daily.
How to Take
Oral administration. Take with water, preferably after meals to minimize gastrointestinal upset.
Mechanism of Action
Nimesulide is a non-steroidal anti-inflammatory drug (NSAID) that primarily acts as a selective inhibitor of cyclooxygenase-2 (COX-2) enzyme. By inhibiting COX-2, it reduces the synthesis of prostaglandins, which are mediators of inflammation, pain, and fever.
Pharmacokinetics
Onset
Pain relief usually starts within 30 minutes.
Excretion
Mainly excreted in urine (about 50-65%) and feces (about 35%).
Half life
Approximately 2-5 hours.
Absorption
Rapidly and well absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 2C9 (CYP2C9), into a main active metabolite, 4'-hydroxy Nimesulide.
Side Effects
Contraindications
- •Hypersensitivity to Nimesulide or other NSAIDs
- •History of gastrointestinal bleeding or peptic ulcer
- •Severe renal impairment
- •Severe hepatic impairment
- •Heart failure
- •Third trimester of pregnancy
- •Children under 12 years
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Warfarin, Heparin
Increased risk of bleeding.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.
Diuretics, ACE inhibitors, Angiotensin II receptor antagonists
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, epigastric pain, nausea, vomiting, gastrointestinal bleeding. In rare cases, anaphylactoid reactions, hypertension, acute renal failure, respiratory depression, and coma may occur. Management involves symptomatic and supportive care. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Contraindicated in the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus and impaired renal function. Should be avoided during early pregnancy and lactation unless clearly necessary and benefits outweigh risks.
