Description
Overview of the medicine
Axet (Cefuroxime Axetil) is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
250-500 mg orally twice daily, depending on the infection.
Elderly
No specific dosage adjustment required based on age, but renal function should be considered.
Renal_impairment
Dosage adjustment necessary for patients with creatinine clearance < 30 mL/min.
How to Take
Take orally, preferably with food to enhance absorption.
Mechanism of Action
Axet inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Antibacterial effect typically observed within hours.
Excretion
Primarily excreted unchanged by glomerular filtration and tubular secretion in the urine.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Well absorbed after oral administration; bioavailability is increased when taken with food. Peak plasma concentrations reached within 2-3 hours.
Metabolism
Cefuroxime Axetil is hydrolyzed by esterases in the intestinal mucosa and blood to Cefuroxime. It is not extensively metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or to any of the excipients.
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Antacids
Drugs that reduce gastric acidity may result in lower bioavailability of Axet and should not be used concurrently.
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Oral Contraceptives
Cefuroxime may affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include convulsions, encephalopathy. Management involves supportive therapy and hemodialysis if necessary.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Use with caution during lactation as cefuroxime is excreted in small amounts in breast milk.

Lab Monitoring
- Periodic assessment of renal function is advised during prolonged therapy, especially in patients with impaired renal function.
- Liver function tests may be monitored in rare cases of hepatic impairment.
Doctor Notes
- Assess patient history for hypersensitivity reactions to beta-lactam antibiotics.
- Monitor for signs of *Clostridioides difficile*-associated diarrhea, which can occur even weeks after treatment.
- Adjust dosage in patients with significant renal impairment.
Patient Guidelines
- Always complete the full course of treatment, even if symptoms improve, to prevent recurrence and resistance.
- Take the medication with food to enhance absorption and reduce gastrointestinal upset.
- Report any signs of severe diarrhea, rash, or allergic reactions to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Axet may cause dizziness. Patients should be warned about driving or operating machinery until they are certain that cefuroxime does not adversely affect their ability to perform such tasks.
Lifestyle Advice
- Maintain good hydration by drinking plenty of fluids.
- Avoid alcohol consumption during the course of treatment, especially if experiencing side effects like dizziness.
