Supadopa TR is an extended-release medication containing levodopa and carbidopa, used to treat Parkinson's disease symptoms such as tremor, rigidity, and slow movement. The extended-release formulation helps to provide a more sustained therapeutic effect.
Uses & Indications
Dosage
Adults
Dosage must be individualized. Typically starting with 100 mg levodopa / 25 mg carbidopa twice daily, adjusted every few days as needed, up to a maximum of 1600 mg levodopa / 400 mg carbidopa per day.
Elderly
Similar to adult dosage, but titration should be done carefully due to increased sensitivity and potential for side effects.
Renal_impairment
No specific dosage adjustment recommended for mild to moderate renal impairment, but caution is advised. Not studied in severe renal impairment.
How to Take
Oral administration. Tablets should be swallowed whole, not crushed, chewed, or divided. Can be taken with or without food, but food may delay absorption.
Mechanism of Action
Levodopa is converted to dopamine in the brain, which helps to alleviate Parkinson's symptoms by replenishing dopamine levels. Carbidopa inhibits peripheral dopa decarboxylase, preventing levodopa breakdown outside the brain, thus increasing the amount of levodopa available to cross the blood-brain barrier and reducing peripheral side effects.

- Concomitant use with non-selective monoamine oxidase (MAO) inhibitors

Iron salts
May reduce the bioavailability of levodopa.
Non-selective MAO inhibitors
May cause hypertensive crisis. Should be discontinued at least 2 weeks prior to initiating levodopa/carbidopa.
Phenothiazines, butyrophenones, reserpine, metoclopramide
May reduce the therapeutic effects of levodopa.
