Uses & Indications
Dosage
Adults
One inhalation (25 mcg) once daily, administered at the same time each day using the specified respirator device.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment, monitor closely.
How to Take

Mechanism of Action
Tiotropium is a long-acting, competitive, and reversible antagonist of muscarinic cholinergic receptors (M1, M2, and M3) located in the airways. It inhibits acetylcholine's bronchoconstrictive effects, leading to bronchodilation. It preferentially binds to M1 and M3 receptors, which mediate bronchoconstriction.

Excretion
Primarily excreted unchanged in urine (about 14% of the dose) and via the fecal route (about 70% of the dose).
Half life
Effective half-life is 25-30 hours, with a terminal half-life of 5-6 days after repeated inhalation in COPD patients.
Absorption
Systemic absorption is low following inhalation, with an absolute bioavailability of 19.5%. Peak plasma concentrations are reached within 5-7 minutes.
Metabolism
Undergoes minimal metabolism via non-enzymatic ester hydrolysis to an inactive alcohol and acid derivative.
Side Effects
Contraindications
- Hypersensitivity to Tiotropium, Atropine, or its derivatives (e.g., Ipratropium, Oxitropium), or to any component of the product.
- Not indicated for the initial treatment of acute episodes of bronchospasm (i.e., rescue therapy).
Drug Interactions
Other anticholinergics
Concomitant administration of other anticholinergic drugs may potentiate adverse effects. Avoid co-administration.
Beta-agonists and corticosteroids
No clinically significant pharmacokinetic interactions have been found when Tiotropium is co-administered with other commonly used COPD medications like sympathomimetic bronchodilators, methylxanthines, and oral or inhaled steroids.

Overdose
Overdose symptoms include anticholinergic effects like dry mouth, blurred vision, and constipation. Treatment is symptomatic and supportive. Close medical supervision is recommended.
