Description
Overview of the medicine
Sustogen 250 mg Injection contains Testosterone Enanthate, an androgen hormone. It is used in males to treat conditions caused by a lack of natural testosterone, such as delayed puberty, impotence, and other hormonal imbalances. It helps restore testosterone levels in the body.
Uses & Indications
Dosage
Adults
For hypogonadism: 50-400 mg intramuscularly every 2 to 4 weeks, depending on clinical response and testosterone levels. For delayed puberty: 50-200 mg every 2-4 weeks for a limited duration.
Elderly
Lower doses may be required, and close monitoring for adverse effects, especially cardiovascular and prostatic issues, is recommended.
Renal_impairment
No specific dose adjustments are typically required, but caution is advised, and monitoring of fluid retention is important.
How to Take
Administer deep intramuscularly. Do not inject intravenously. The injection site should be alternated to avoid scarring and pain. Administer slowly.
Mechanism of Action
Testosterone Enanthate is an ester of the androgen testosterone. It acts by binding to androgen receptors in target tissues, leading to the activation of specific genes and the synthesis of proteins, which in turn promotes the development and maintenance of male secondary sexual characteristics and other androgenic effects. The enanthate ester allows for a sustained release of testosterone.
Pharmacokinetics
Onset
Clinical effects typically begin within weeks to months, with stable levels reached after several doses.
Excretion
Excreted primarily in the urine (about 90%) and feces (about 10%) as conjugated metabolites.
Half life
The half-life of testosterone enanthate itself is about 4.5 days, while the effective half-life of testosterone after hydrolysis is much shorter, around 10-100 minutes, but due to slow release, it provides sustained levels for 2-4 weeks.
Absorption
Administered intramuscularly, testosterone enanthate is slowly released from the injection site and hydrolyzed to testosterone. Peak plasma levels are reached within 2-4 days.
Metabolism
Metabolized primarily in the liver by various enzymes (e.g., CYP3A4) to inactive metabolites like androsterone and etiocholanolone, and also to active metabolites like dihydrotestosterone (DHT) and estradiol.
Side Effects
Contraindications
- Known or suspected carcinoma of the breast in men
- Known or suspected carcinoma of the prostate
- Pregnancy and lactation
- Hypersensitivity to testosterone or any component of the formulation
- Severe cardiac, renal, or hepatic disease
Drug Interactions
Corticosteroids
Concurrent use may increase the risk of edema.
Thyroid Hormones
May decrease serum concentrations of thyroxine-binding globulin, leading to increased free thyroid hormone levels.
Anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, requiring dose adjustment and increased monitoring of INR.
Insulin/Oral Hypoglycemic Agents
Testosterone may decrease blood glucose, requiring dose adjustment of antidiabetic drugs.
Storage
Store at controlled room temperature (20-25°C), excursions permitted to 15-30°C. Protect from light. Do not refrigerate or freeze.
Overdose
Symptoms may include virilization, gynecomastia, fluid retention, polycythemia, and psychiatric effects. Treatment is symptomatic and supportive. Discontinue the drug if symptoms of overdose occur.
Pregnancy & Lactation
Contraindicated in pregnant and breastfeeding women due to potential for virilization of the fetus or infant. Category X for pregnancy.

Lab Monitoring
- Serum testosterone levels (before and periodically during treatment)
- Hematocrit and hemoglobin levels (due to risk of polycythemia)
- Liver function tests (LFTs)
- Lipid profile
- Prostate-specific antigen (PSA) in men over 40 (due to risk of prostatic issues)
Doctor Notes
- Careful patient selection and monitoring are crucial, especially for older patients and those with pre-existing cardiovascular risk factors or prostate issues.
- Discuss potential for abuse with patients.
- Ensure patients understand the need for lifelong treatment in most cases of hypogonadism.
