Overview of the medicine
Suxen-10-W Gel is a topical preparation containing Aceclofenac, an NSAID, and Methyl Salicylate, a counter-irritant. It is used for localized pain and inflammation associated with musculoskeletal conditions.
Uses & Indications
Dosage
Adults
Apply a thin layer of gel to the affected area 2-3 times daily, or as directed by the physician.
Elderly
No special dosage adjustment is required for elderly patients; use as for adults.
Renal_impairment
No specific dosage adjustment is required for topical application in patients with renal impairment.
How to Take
For external use only. Apply a thin layer evenly to the intact skin over the affected area. Gently rub until absorbed. Wash hands thoroughly after application. Do not apply to open wounds, infected skin, eyes, or mucous membranes.
Mechanism of Action
Aceclofenac inhibits prostaglandin synthesis by blocking cyclooxygenase (COX) enzymes, thereby reducing inflammation, pain, and fever. Methyl Salicylate acts as a counter-irritant, producing a sensation of warmth that distracts from the pain and may cause local vasodilation, aiding blood flow to the affected area.

Onset
Local counter-irritant effect within minutes; analgesic and anti-inflammatory effects develop over a few hours.
Excretion
Systemically absorbed aceclofenac metabolites are excreted in urine and faeces. Salicylic acid metabolites are primarily excreted renally.
Half life
Systemic half-life of aceclofenac is about 4 hours. Methyl salicylate is rapidly metabolized to salicylic acid.
Absorption
Minimal systemic absorption after topical application; primarily acts locally. A small fraction of aceclofenac and methyl salicylate may be absorbed systemically.
Metabolism
Aceclofenac undergoes extensive metabolism to its hydroxylated derivatives. Methyl salicylate is metabolized to salicylic acid, primarily in the skin and liver.
Side Effects
Contraindications
- Hypersensitivity to aceclofenac, methyl salicylate, aspirin, or any other NSAID.
- Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
- Application to open wounds, damaged skin, infected areas, eyes, or mucous membranes.
- Third trimester of pregnancy.

Concomitant use with oral NSAIDs may increase the risk of systemic adverse effects, though unlikely with typical topical application.
Lithium and Methotrexate
Increased plasma concentrations and toxicity are theoretically possible if significant systemic absorption occurs, but highly improbable with normal topical use.
Oral Anticoagulants (e.g., Warfarin)
Although systemic absorption is minimal, theoretical risk of increased bleeding due to additive effects, especially with prolonged or extensive use.
