Description
Overview of the medicine
Desvenlafaxine is an antidepressant used to treat Major Depressive Disorder (MDD). It helps restore the balance of certain natural substances (serotonin and norepinephrine) in the brain, improving mood and other symptoms of depression.
Uses & Indications
Dosage
Adults
The recommended starting dose is 50 mg once daily. Dosage may be increased to 75 mg, 100 mg, 200 mg, or up to 400 mg once daily based on clinical response and tolerability. The 75 mg ODT is often used when a higher starting dose or an intermediate dose is desired.
Elderly
No specific dose adjustment is required, but caution is advised due to potential age-related decrease in renal clearance. Start with a lower dose and monitor carefully.
Renal_impairment
Moderate impairment (CrCl 30-50 mL/min): 50 mg every other day. Severe impairment (CrCl <30 mL/min) or End-Stage Renal Disease (ESRD) on hemodialysis: 50 mg twice weekly.
How to Take
Place the orally disintegrating tablet on the tongue and allow it to dissolve completely, then swallow with or without water. Take once daily, with or without food, at approximately the same time each day.
Mechanism of Action
Desvenlafaxine works by selectively inhibiting the reuptake of serotonin and norepinephrine in the brain, thereby increasing the availability of these neurotransmitters in the synaptic cleft, which helps to improve mood and reduce symptoms of depression.

Metabolism
Primarily undergoes O-demethylation (minor contribution from CYP3A4). It is not extensively metabolized by CYP450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to desvenlafaxine or any component of the formulation.
- Concomitant use with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
Drug Interactions
Alcohol
Avoid concurrent use, as it may enhance CNS depressant effects.
Monoamine Oxidase Inhibitors (MAOIs)
Serious, potentially fatal reactions including serotonin syndrome. Do not use within 14 days of stopping an MAOI, or within 7 days of stopping desvenlafaxine before starting an MAOI.
Drugs that affect hemostasis (e.g., NSAIDs, aspirin, warfarin)
Increased risk of bleeding.
Serotonergic drugs (e.g., SSRIs, other SNRIs, triptans, tramadol, St. John's wort)
