Description
Overview of the medicine
Coport L is a novel broad-spectrum antibacterial agent indicated for the treatment of various bacterial infections including respiratory tract infections, urinary tract infections, and skin and soft tissue infections. It works by inhibiting bacterial protein synthesis, leading to bacterial cell death.
Uses & Indications
Dosage
Adults
500 mg orally twice daily for 7-14 days, depending on the infection.
Elderly
No specific dose adjustment is required unless renal impairment is present. Monitor renal function.
Renal_impairment
For CrCl 30-50 mL/min, 500 mg once daily. For CrCl <30 mL/min, 250 mg once daily or alternate day dosing. Consult physician.
How to Take
Take orally with or without food. For best absorption, it is recommended to take it with a glass of water. Do not crush or chew the tablet.
Mechanism of Action
Coport L inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby blocking peptide chain elongation. This leads to a bacteriostatic effect at lower concentrations and a bactericidal effect at higher concentrations.
Pharmacokinetics
Onset
Onset of antibacterial action typically within 1 hour after administration.
Excretion
Mainly excreted through urine (60-70% as unchanged drug and metabolites) and some biliary excretion.
Half life
Approximately 6-8 hours, allowing for twice-daily dosing.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via oxidative pathways, producing inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Coport L or any component of the formulation.
- Severe hepatic impairment.
- History of cholestatic jaundice/hepatic dysfunction associated with previous Coport L use.
Drug Interactions
Digoxin
May increase digoxin levels; monitor digoxin concentration.
Antacids
May decrease absorption of Coport L if taken concomitantly; administer Coport L 2 hours before or 6 hours after antacids.
Warfarin
May increase anticoagulant effect; monitor INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and dizziness. In case of overdose, seek immediate medical attention. Management is primarily supportive; activated charcoal may be considered within 1 hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C: Use only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; use with caution in nursing mothers.
