Overview of the medicine
Syp-Zi 10 mg/mL Syrup contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Adult dosage varies by indication; typically 500 mg once daily for 3 days or 500 mg on day 1 followed by 250 mg once daily for 4 days. This syrup formulation is primarily for pediatric use.
Elderly
No specific dose adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment.
How to Take
Shake the bottle well before each use. Administer orally, preferably on an empty stomach (1 hour before or 2 hours after food) for optimal absorption. However, it can be taken with food to minimize gastrointestinal upset. Complete the full course of treatment.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis. This ultimately interferes with the bacterium's ability to multiply and spread.

Excretion
Mainly excreted unchanged in bile, a small percentage is excreted in urine.
Half life
Approximately 2-4 days (terminal half-life).
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37% when taken orally. Absorption is decreased by food.
Metabolism
Primarily hepatic, with no significant first-pass metabolism. Metabolites are not thought to be microbiologically active.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin or any macrolide or ketolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin serum concentrations; monitor for known azithromycin side effects.
Antacids (aluminum/magnesium containing)
Decreased peak plasma concentration of azithromycin, administer azithromycin at least 1 hour before or 2 hours after antacids.
QT-prolonging drugs (e.g., amiodarone, sotalol)
Increased risk of cardiac arrhythmias.
Storage
Store the powder below 30°C, protected from light and moisture. After reconstitution, store the suspension at room temperature (below 30°C) and discard any unused portion after the recommended period.
Overdose
Symptoms of overdose are typically an exaggeration of known side effects, including severe nausea, vomiting, diarrhea, and reversible hearing loss. Management should be symptomatic and supportive; gastric lavage and general supportive measures are recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Excreted in breast milk, caution advised when administering to a nursing mother.
