Description
Overview of the medicine
Systaflam Topical Gel is an analgesic and anti-inflammatory medicine used for the relief of pain and inflammation in muscles, joints, and soft tissues.
Uses & Indications
Dosage
Adults
Apply 2-4 grams of gel (corresponding to a 2-inch strip) to the affected area 2 to 4 times daily, rubbing gently until absorbed. The duration of treatment depends on the indication and response.
Elderly
No specific dose adjustment is required for elderly patients. Use as for adults.
Renal_impairment
No dose adjustment is necessary as systemic absorption is minimal.
How to Take
For external use only. Apply a small amount of gel to the affected area and gently rub it into the skin. Wash hands thoroughly after application, unless hands are the treated area.
Mechanism of Action
Diclofenac, a non-steroidal anti-inflammatory drug (NSAID), inhibits prostaglandin synthesis, reducing pain and inflammation. Methyl Salicylate and Menthol act as counterirritants, providing a cooling and warming sensation that distracts from pain and increases local blood flow.
Pharmacokinetics
Onset
Local analgesic effect typically observed within 15-30 minutes of application.
Excretion
Systemically absorbed diclofenac and its metabolites are primarily excreted via urine and bile.
Half life
Systemic half-life of absorbed diclofenac is approximately 1-2 hours; local therapeutic effect is prolonged.
Absorption
Minimal systemic absorption; primarily acts locally. A small fraction of diclofenac is absorbed systemically through the skin.
Metabolism
Primarily localized metabolism in the skin. Systemically absorbed diclofenac is metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, methyl salicylate, menthol, linseed oil, or any other components of the gel.
- Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
- Application on broken or damaged skin, open wounds, or infections.
- Use in the third trimester of pregnancy due to potential fetal harm.
Drug Interactions
Oral NSAIDs
Although systemic absorption is minimal, concurrent use with oral NSAIDs may increase the risk of systemic adverse effects, especially in patients with pre-existing conditions.
Storage
Store below 30°C in a cool, dry place, protected from light and moisture. Do not freeze. Keep away from children.
Overdose
Overdose with topical application is unlikely to cause systemic toxicity due to poor absorption. Excessive use may lead to local skin irritation, redness, or burning. Treatment involves washing off the excess gel and symptomatic management.
Pregnancy & Lactation
Use with caution during pregnancy, especially in the first two trimesters, and avoid entirely in the third trimester. Consult a doctor before use during breastfeeding, as diclofenac may pass into breast milk in small amounts.
