Description
Overview of the medicine
Talmimax contains Telmisartan, an angiotensin II receptor blocker (ARB), used to treat high blood pressure and reduce cardiovascular risk.
Uses & Indications
Dosage
Adults
Usual starting dose is 40 mg once daily. Dose may be increased to a maximum of 80 mg once daily if needed.
Elderly
No dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Can be taken with or without food. Administer once daily at approximately the same time.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor, which inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect observed within 3 hours, full effect within 4-8 weeks.
Excretion
Excreted almost exclusively in feces via bile, mainly as unchanged drug.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations reached in 0.5-1 hour. Absolute bioavailability is dose-dependent (approx. 42% for 40mg, 58% for 80mg).
Metabolism
Minimally metabolized; primarily conjugated to glucuronide.

Contraindications
- Hypersensitivity to Telmisartan or any excipient
- Pregnancy (especially 2nd and 3rd trimesters)
- Lactation
- Biliary obstructive disorders
- Severe hepatic impairment
- Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 ml/min/1.73m²)
Drug Interactions
Digoxin
Increased plasma digoxin levels.
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics (e.g., Furosemide)
Increased risk of hypotension.
NSAIDs (e.g., Ibuprofen, Indomethacin)
May reduce antihypertensive effect and increase risk of renal impairment.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
