Description
Overview of the medicine
Tamcontin 0.4 mg tablet is used to treat the signs and symptoms of benign prostatic hyperplasia (BPH), such as difficulty urinating, weak stream, and frequent urination.
Uses & Indications
Dosage
Adults
0.4 mg once daily, approximately 30 minutes after the same meal each day.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl < 10 mL/min).
How to Take
Take orally, once daily, approximately 30 minutes after the same meal each day. Swallow the tablet whole; do not crush, chew, or open it.
Mechanism of Action
Tamsulosin is an alpha-1 adrenergic blocker that relaxes the smooth muscles in the prostate and bladder neck, which improves urine flow and reduces BPH symptoms.
Pharmacokinetics
Onset
Symptomatic improvement may take several days to weeks.
Excretion
Primarily excreted in the urine (approximately 76%) and feces (approximately 21%) as metabolites.
Half life
Approximately 10-13 hours.
Absorption
Well absorbed orally, with approximately 90% bioavailability. Absorption is affected by food.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (primarily CYP3A4 and CYP2D6) in the liver.
Side Effects
Contraindications
- Hypersensitivity to tamsulosin hydrochloride or any component of the formulation.
- History of orthostatic hypotension.
- Severe hepatic impairment.
Drug Interactions
PDE5 Inhibitors
Co-administration with phosphodiesterase-5 (PDE5) inhibitors (e.g., sildenafil, tadalafil) may result in additive blood pressure lowering effects, leading to symptomatic hypotension.
Strong CYP3A4 Inhibitors
Co-administration with strong CYP3A4 inhibitors (e.g., ketoconazole) may increase tamsulosin exposure, leading to increased risk of adverse effects.
Other Alpha-Adrenergic Blockers
Increased risk of hypotension when used concurrently with other alpha-adrenergic blocking agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to severe hypotensive effects. Management involves supportive measures, including keeping the patient supine and administering vasopressors if necessary. Dialysis is unlikely to be beneficial.
Pregnancy & Lactation
Tamsulosin is not indicated for use in women. Risks during pregnancy and lactation are unknown if used off-label.

Clinical Trials
Tamsulosin has undergone extensive clinical trials demonstrating its efficacy and safety in treating BPH symptoms. Post-marketing surveillance continues to monitor its long-term effects.
Lab Monitoring
- No routine laboratory monitoring is required. Blood pressure should be monitored regularly, especially for orthostatic changes.
Doctor Notes
- Counsel patients on the potential for orthostatic hypotension and ejaculatory dysfunction.
- Advise patients to inform ophthalmologists about tamsulosin use prior to cataract or glaucoma surgery due to the risk of Intraoperative Floppy Iris Syndrome (IFIS).
- Monitor blood pressure regularly, especially in patients initiating therapy or undergoing dose adjustments.
Patient Guidelines
- Take the medicine consistently at the same time each day after a meal.
- Avoid sudden changes in body position (e.g., standing up quickly) to minimize dizziness or lightheadedness.
- Inform your doctor if you are scheduled for cataract or glaucoma surgery, as tamsulosin can cause Intraoperative Floppy Iris Syndrome (IFIS).
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for a missed one.
Driving Precautions
May cause dizziness or lightheadedness, particularly at the initiation of therapy or during dose changes. Patients should be cautioned about operating hazardous machinery, including automobiles.
Lifestyle Advice
- Avoid activities requiring mental alertness, such as driving or operating machinery, if you experience dizziness or lightheadedness, especially at the start of treatment.
- Maintain adequate fluid intake to support overall urinary health.
