Tamen 10 mg Injection contains Tramadol Hydrochloride, an opioid analgesic used to relieve moderate to moderately severe pain. It works by binding to opioid receptors in the brain and spinal cord, altering the perception of pain.
Uses & Indications
Dosage
Adults
Typically, 50-100 mg by slow intravenous (IV) injection over 2-3 minutes, intramuscular (IM) injection, or subcutaneous (SC) injection every 4-6 hours as needed. Maximum daily dose usually 400 mg. For a 10 mg/mL vial, multiple vials might be required per dose.
Elderly
Reduced dosage may be required, especially in those over 75 years, usually 50 mg every 6 hours, not exceeding 300 mg daily. Dosage should be adjusted based on renal function.
Renal_impairment
Creatinine clearance < 30 mL/min: dosage interval should be extended to 12 hours, maximum daily dose not exceeding 200 mg.
How to Take
Administer by slow intravenous (IV) injection over 2-3 minutes, or as an infusion. Can also be given intramuscularly (IM) or subcutaneously (SC).
Mechanism of Action
Tramadol acts as a centrally acting synthetic opioid analgesic. It has a dual mechanism of action: it binds to μ-opioid receptors, and it also inhibits the reuptake of norepinephrine and serotonin, which contributes to its analgesic effect.

Onset
Within 15-30 minutes after intravenous administration.
Excretion
Mainly excreted by the kidneys (approximately 90%), with a small amount excreted in feces (approximately 10%).
Half life
Approximately 6-7 hours for the parent drug and 7.9 hours for the active metabolite O-desmethyltramadol (M1).
Absorption
Rapidly and almost completely absorbed after parenteral administration. Bioavailability is high.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP3A4, to several metabolites including the active O-desmethyltramadol (M1).
Side Effects
Contraindications
- Acute intoxication with alcohol, hypnotics, analgesics, opioids, psychotropic drugs
- Patients receiving MAO inhibitors or who have taken them within the last 14 days
- Epilepsy not controlled by treatment
- Severe respiratory depression
Drug Interactions
Carbamazepine
Significantly decreases tramadol concentration, reducing analgesic effect.
MAO Inhibitors
Concomitant use is contraindicated due to risk of serotonin syndrome and seizures.
Serotonergic Drugs (e.g., SSRIs, SNRIs, TCAs)
Increased risk of serotonin syndrome.
CNS Depressants (e.g., benzodiazepines, alcohol, other opioids)
Increased risk of respiratory depression, profound sedation, coma, and death.
Storage
Store at room temperature (below 30°C), away from light and moisture. Do not freeze.
Overdose
Symptoms include respiratory depression, seizures, miosis, cardiovascular collapse, and coma. Treatment involves supportive measures, maintaining a patent airway, and administration of naloxone for opioid effects. Gastric lavage may be considered if recent oral ingestion (not applicable for injection directly).

Pregnancy Category C. Not recommended during pregnancy, especially in the third trimester, due to risk of neonatal opioid withdrawal syndrome. Small amounts are excreted in breast milk; not recommended for nursing mothers.
