Description
Overview of the medicine
Tamsin D is a combination medicine used to treat symptoms of benign prostatic hyperplasia (BPH) in men with an enlarged prostate. It helps to relax the muscles in the prostate and bladder, improving urine flow, and also reduces the size of the prostate gland over time.
Uses & Indications
Dosage
Adults
One capsule (0.4 mg Tamsulosin + 0.5 mg Dutasteride) once daily, taken approximately 30 minutes after the same meal each day.
Elderly
No specific dosage adjustment is required for elderly patients, but caution is advised due to potential for orthostatic hypotension.
Renal_impairment
No dosage adjustment is needed for mild to moderate renal impairment. Use with caution in severe renal impairment (creatinine clearance <10 mL/min).
How to Take
Take the capsule orally, once daily, approximately 30 minutes after the same meal each day. Swallow the capsule whole; do not chew, crush, or open it, as this may affect the sustained-release properties and potentially increase side effects.
Mechanism of Action
Tamsulosin selectively blocks alpha-1A adrenergic receptors in the prostate, bladder neck, and urethra, leading to relaxation of smooth muscles and improved urine flow. Dutasteride inhibits both type 1 and type 2 5-alpha reductase isoenzymes, preventing the conversion of testosterone to dihydrotestosterone (DHT). Reduced DHT levels lead to a decrease in prostate size.
Pharmacokinetics
Onset
Tamsulosin: Within a few hours for symptomatic relief. Dutasteride: Several weeks to months for prostate size reduction.
Excretion
Tamsulosin: Primarily excreted in the urine. Dutasteride: Primarily excreted in feces, with a small amount in urine.
Half life
Tamsulosin: Approximately 5-10 hours. Dutasteride: Approximately 3-5 weeks.
Absorption
Tamsulosin is well absorbed from the gastrointestinal tract. Dutasteride is well absorbed, reaching peak serum concentrations within 1-3 hours.
Metabolism
Both tamsulosin and dutasteride are extensively metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to tamsulosin, dutasteride, other 5-alpha reductase inhibitors, or any component of the formulation
- Women, especially those who are pregnant or may become pregnant, and children
- Severe hepatic impairment
Drug Interactions
Warfarin
Dutasteride may displace warfarin from protein binding sites, requiring monitoring of INR.
PDE5 inhibitors (e.g., sildenafil, tadalafil)
Co-administration with alpha-blockers like tamsulosin may potentiate hypotensive effects.
Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
May increase tamsulosin exposure, potentially leading to increased risk of hypotension.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose with Tamsin D may lead to severe hypotension due to the tamsulosin component. Symptoms may include dizziness, fainting, and weakness. Management involves symptomatic and supportive treatment, including monitoring blood pressure, fluid replacement, and vasopressors if necessary.
Pregnancy & Lactation
Tamsin D is contraindicated in women, especially those who are pregnant or may become pregnant, and during lactation. Dutasteride can cause male fetal abnormalities. Pregnant women should not handle the capsules due to the risk of absorption.

Clinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of Tamsin D in improving BPH symptoms, urine flow, and reducing prostate volume, as well as lowering the risk of acute urinary retention and BPH-related surgery.
Lab Monitoring
- Prostate-specific antigen (PSA) levels should be measured before initiating treatment and periodically thereafter. Dutasteride can lower PSA levels, potentially masking prostate cancer detection. Liver function tests should be performed periodically.
Doctor Notes
- Advise patients about the potential for orthostatic hypotension, particularly at the initiation of therapy. Counsel on the importance of taking the medication consistently after a meal.
- Emphasize that women, especially pregnant women, must not handle the capsules due to the risk of absorption of dutasteride and potential harm to a male fetus.
- Monitor PSA levels as dutasteride lowers PSA. A baseline PSA value should be established before treatment.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor, at the same time each day, after a meal.
- Swallow the capsule whole; do not chew, crush, or open it.
- Be aware of potential dizziness, especially when standing up from a sitting or lying position. Stand up slowly.
- Women who are pregnant or may become pregnant should not handle these capsules.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Tamsin D may cause dizziness or lightheadedness, especially at the start of treatment or when changing dose. Avoid driving or operating machinery until you know how this medicine affects you.
Lifestyle Advice
- Maintain a healthy lifestyle, including regular exercise and a balanced diet.
- Stay well-hydrated.
- Avoid excessive alcohol consumption, which can worsen BPH symptoms or orthostatic hypotension.
- Regularly monitor your BPH symptoms and consult your doctor for any concerns.
