Description
Overview of the medicine
Tanpot 50 mg Tablet contains Atenolol, a selective beta-1 adrenergic blocker, used to treat high blood pressure, angina pectoris, and certain cardiac arrhythmias. It works by reducing the heart rate and the force of contraction.
Uses & Indications
Dosage
Adults
Hypertension: Initially 50 mg once daily, may be increased to 100 mg once daily after 1-2 weeks. Angina: 50 mg once daily, may be increased to 100 mg once daily. Arrhythmias: 50-100 mg once daily. Acute MI: Initial oral 50 mg, then 50 mg after 12 hours, then 100 mg once daily.
Elderly
Lower doses may be sufficient. Careful monitoring of blood pressure and heart rate is recommended. Start with 25 mg once daily.
Renal_impairment
Dosage adjustment required. Creatinine clearance 15-35 mL/min: 50 mg daily or 100 mg every other day. Creatinine clearance <15 mL/min: 50 mg every other day or 100 mg every 4 days.
How to Take
Take orally, with or without food, preferably at the same time each day. Do not crush or chew the tablet.
Mechanism of Action
Atenolol selectively blocks beta-1 adrenergic receptors, primarily in the heart. This reduces heart rate, myocardial contractility, and cardiac output, leading to a decrease in blood pressure. It also reduces renin release from the kidneys.
Pharmacokinetics
Onset
Within 1 hour; full effect within 1-2 weeks of regular dosing.
Excretion
Mainly by renal excretion (about 50% unchanged in urine).
Half life
6-7 hours (prolonged in renal impairment).
Absorption
Approximately 50% absorbed orally. Peak plasma concentrations reached within 2-4 hours.
Metabolism
Minimal hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Bradycardia (slow heart rate)
- Second or third-degree heart block
- Cardiogenic shock
- Uncontrolled heart failure
- Sick sinus syndrome
- Severe peripheral arterial disease
Drug Interactions
NSAIDs
May reduce the antihypertensive effect of Atenolol.
Digoxin
Increased risk of bradycardia.
Clonidine
Risk of rebound hypertension if Clonidine is withdrawn abruptly while taking Atenolol.
Insulin/Oral Hypoglycemics
May mask symptoms of hypoglycemia and alter glucose levels.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and heart block.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe bradycardia, hypotension, acute cardiac failure, bronchospasm. Management involves gastric lavage, activated charcoal, and supportive measures like atropine for bradycardia, glucagon, IV fluids, and vasopressors for hypotension.
Pregnancy & Lactation
Pregnancy Category D. Atenolol crosses the placenta and has been associated with intrauterine growth retardation and neonatal bradycardia. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk; avoid use during lactation or monitor infant for beta-blocking effects.
