Description
Overview of the medicine
Tarcenib 100 mg Tablet is an anti-cancer medication used in the treatment of certain types of non-small cell lung cancer and pancreatic cancer. It works by blocking the action of an abnormal protein that signals cancer cells to multiply.
Uses & Indications
Dosage
Adults
NSCLC: 150 mg orally once daily. Pancreatic cancer: 100 mg orally once daily in combination with gemcitabine.
Elderly
No specific dose adjustment required based on age.
Renal_impairment
No specific dose adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally at least one hour before or two hours after a meal. Swallow the tablet whole with water. Do not crush, cut, or chew.
Mechanism of Action
Tarcenib is a small molecule tyrosine kinase inhibitor. It specifically targets the epidermal growth factor receptor (EGFR) by reversibly binding to the ATP-binding site of the receptor's tyrosine kinase. This inhibits autophosphorylation of EGFR, leading to the inhibition of downstream signaling pathways involved in cell proliferation, survival, and angiogenesis.
Pharmacokinetics
Onset
Clinical effects may take several weeks to manifest.
Excretion
Mainly excreted via feces (approximately 80%), with a small amount (around 10%) excreted renally.
Half life
Approximately 36 hours (range 20-50 hours).
Absorption
Oral absorption, approximately 60% bioavailable. Peak plasma concentrations reached within 4 hours.
Metabolism
Primarily hepatic metabolism via cytochrome P450 enzymes (CYP3A4, some CYP1A2).
Side Effects
Contraindications
- Hypersensitivity to tarcenib or any excipients
- Severe hepatic impairment (Child-Pugh C)
Drug Interactions
CYP3A4 Inducers (e.g., rifampicin, phenytoin)
May decrease Tarcenib plasma concentrations, reducing efficacy. Avoid concomitant use.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase Tarcenib plasma concentrations, increasing risk of side effects. Dose adjustment may be necessary.
Proton Pump Inhibitors (PPIs) & H2-receptor antagonists
May decrease Tarcenib solubility and absorption due to increased gastric pH. Avoid concomitant use or separate administration.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited information on Tarcenib overdose. Doses up to 1600 mg have been observed to be well tolerated in single dose studies. Overdose may increase frequency and severity of adverse effects, particularly skin rash, diarrhea, and potentially more serious events like ILD. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category D. Tarcenib can cause fetal harm. Advise pregnant women of the potential risk to a fetus. Advise women of reproductive potential to use effective contraception. It is unknown if Tarcenib is excreted in human milk; avoid breastfeeding during treatment.
