Description
Overview of the medicine
Tav (Levofloxacin) is a broad-spectrum fluoroquinolone antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typically 250 mg to 750 mg once daily, depending on the indication and severity of infection. Duration varies from 3 to 14 days.
Elderly
No dose adjustment generally required based on age alone; however, renal function should be assessed and dose adjusted if impaired.
Renal_impairment
Dose adjustment is necessary for patients with creatinine clearance <50 mL/min. Refer to product insert for specific guidelines.
How to Take
Tav tablets can be taken orally with or without food. It is important to drink plenty of fluids while taking this medication. Do not take antacids or iron supplements within two hours of taking Tav.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Variable, depending on the type and severity of infection.
Excretion
Primarily excreted unchanged in the urine (over 85% of dose) via glomerular filtration and tubular secretion.
Half life
Approximately 6-8 hours, allowing for once-daily dosing.
Absorption
Rapid and complete absorption after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized in the liver (less than 5% of dose).
Side Effects
Contraindications
- Hypersensitivity to levofloxacin, other quinolones, or any component of the formulation.
- History of tendon disorders related to fluoroquinolone administration.
- Epilepsy (due to increased risk of convulsions).
Drug Interactions
0
Antacids, Sucralfate, Metal Cations (e.g., iron, zinc): Reduced oral absorption of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
1
Warfarin: Enhanced anticoagulant effect. Monitor INR closely.
2
Corticosteroids: Increased risk of tendon rupture.
3
NSAIDs: Increased risk of CNS stimulation and convulsive seizures.
4
Antidiabetic agents (e.g., insulin, oral hypoglycemics): Disturbances in blood glucose, including hyperglycemia and hypoglycemia.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include central nervous system symptoms (e.g., confusion, dizziness, impaired consciousness, convulsive seizures), QT prolongation. Management includes symptomatic and supportive treatment, including ECG monitoring. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Levofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Doctor Notes
- Caution should be exercised in elderly patients and those with renal impairment, diabetes, epilepsy, or myasthenia gravis.
- Patients should be advised to report any signs of tendon pain, neuropathy, or severe diarrhea immediately.
- Avoid co-administration with multivalent cations.
Patient Guidelines
- Complete the full course of treatment, even if you feel better, to prevent recurrence and resistance.
