Description
Overview of the medicine
Cefpodoxime proxetil is an oral third-generation cephalosporin antibiotic. It is a prodrug that is hydrolyzed to cefpodoxime in vivo. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Not typically used for adults at this strength; adult dosage for Cefpodoxime is typically 100-200 mg every 12 hours for tablet forms. If used for adults, dose would be adjusted.
Elderly
No specific dose adjustment necessary unless severe renal impairment. Refer to adult dosage guidelines with caution for elderly with normal renal function.
Renal_impairment
Adjust dose for creatinine clearance < 30 mL/min; administer every 24 hours. For children, consult a physician.
How to Take
Shake the bottle well before each use. Administer orally using the provided measuring spoon or dropper. It should be taken with food to enhance absorption.
Mechanism of Action
Cefpodoxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Typically within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine, with about 29-33% of the administered dose recovered in the urine within 12 hours.
Half life
Approximately 2.0-3.0 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 50% and increases with food.
Metabolism
Cefpodoxime proxetil is a prodrug that is hydrolyzed by esterases in the gut wall to the active metabolite cefpodoxime.
Side Effects
Contraindications
- Hypersensitivity to cefpodoxime, other cephalosporins, or any component of the formulation
- History of anaphylactic reactions to penicillin
Drug Interactions
Probenecid
May increase cefpodoxime levels.
Nephrotoxic drugs (e.g., aminoglycosides)
May increase risk of nephrotoxicity (though less common with cefpodoxime).
Antacids/H2 blockers/Proton Pump Inhibitors
May decrease absorption of cefpodoxime.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and discard after 10 days.
Overdose
Symptoms may include nausea, vomiting, epigastric distress, and diarrhea. Management is primarily supportive and symptomatic. Hemodialysis can remove cefpodoxime from the system.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.
