Description
Overview of the medicine
Tdcef-DS 200 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is primarily used to treat a wide range of bacterial infections including those affecting the respiratory tract, urinary tract, and ears. This suspension form is particularly suitable for pediatric patients.
Uses & Indications
Dosage
Adults
Not typically recommended for adults in suspension form; usually 200-400 mg daily as tablet/capsule.
Elderly
Dosage adjustment may not be necessary unless severe renal impairment.
Children
8 mg/kg/day orally, as a single dose or in two divided doses for 7-14 days. Maximum 400 mg/day.
Renal_impairment
Dosage adjustment required for creatinine clearance < 60 mL/min.
How to Take
For oral administration. The dry powder should be reconstituted with boiled and cooled water as per instructions on the label. Shake well before each use. Can be taken with or without food.
Mechanism of Action
Cefixime exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations usually occur within 2-6 hours after oral administration.
Excretion
Renal excretion (about 50% of absorbed dose), some biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed, food may delay but not significantly alter absorption. Peak plasma concentrations usually occur within 2-6 hours.
Metabolism
Not extensively metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase the anticoagulant effect, requiring close monitoring of INR.
Carbamazepine
May increase carbamazepine plasma levels.
Oral Contraceptives
Theoretical risk of reduced efficacy of oral contraceptives (though less commonly observed).
Storage
Store dry powder below 30°C in a dry place, away from direct sunlight and moisture. Once reconstituted, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period (e.g., 7 or 14 days).
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be supportive and symptomatic. Hemodialysis and peritoneal dialysis do not remove significant quantities of cefixime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, it should be used during pregnancy only if clearly needed. Cefixime is excreted in human milk in small quantities; caution should be exercised when administered to a nursing mother.

- Complete blood count (CBC) during prolonged use.
- Liver function tests if clinically indicated.
Doctor Notes
- Counsel patients/parents on the importance of completing the full course of antibiotics to prevent resistance and recurrence.
