Overview of the medicine
Tekna (Paracetamol) is a widely used over-the-counter medication for the relief of mild to moderate pain and fever. It belongs to the class of non-opioid analgesics and antipyretics.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed. Maximum 4000 mg in 24 hours.
Elderly
Usually the same as adults, but caution for renal/hepatic impairment.
Renal_impairment
Dosage adjustment may be required; consult a physician. Extended dosing interval for severe impairment.
How to Take
Take orally with or without food. Do not exceed the recommended dose.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, increasing the pain threshold by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are involved in prostaglandin synthesis. Its antipyretic effect is due to direct action on the heat-regulating centers in the hypothalamus, leading to peripheral vasodilation and sweating.
Pharmacokinetics
Onset
10 to 30 minutes (analgesia), 30 to 60 minutes (antipyresis)
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1 to 3 hours, increased in hepatic impairment and overdose.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 10 to 60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by cytochrome P450 to a toxic intermediate, NAPQI, which is normally detoxified by glutathione.
