Description
Overview of the medicine
Telmisartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in certain high-risk patients. It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Usually 40 mg once daily. May be increased to a maximum of 80 mg once daily.
Elderly
No dosage adjustment necessary.
Renal_impairment
No dosage adjustment necessary for mild to moderate impairment. Not recommended for severe impairment or hemodialysis.
How to Take
Oral administration, once daily with or without food.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.

Absorption
Rapidly absorbed from the gastrointestinal tract. Absolute bioavailability is dose-dependent, approx. 42% for 40 mg and 58% for 160 mg.
Metabolism
Minimally metabolized by conjugation to form an acylglucuronide; no cytochrome P450 involvement.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Second and third trimesters of pregnancy
- Biliary obstructive disorders
- Severe hepatic impairment
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
NSAIDs
May reduce the antihypertensive effect of telmisartan and increase the risk of renal function deterioration.
Digoxin
Increased plasma digoxin levels.
Lithium
Increased serum lithium concentrations and toxicity have been reported.
Diuretics
Increased risk of hypotension and renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Hypotension, dizziness, tachycardia, bradycardia. Management: Symptomatic and supportive treatment. Hemodialysis is not effective.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to fetal toxicity. Not recommended during the first trimester. Breastfeeding is not recommended.
