Overview of the medicine
Telcardis Plus 80 mg Tablet is a combination medicine used to treat high blood pressure (hypertension). It contains Telmisartan, an angiotensin receptor blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic. This combination helps to lower blood pressure more effectively than either component alone, reducing the risk of cardiovascular events.
Uses & Indications
Dosage
Adults
Usually one tablet (Telmisartan 80 mg / Hydrochlorothiazide 12.5 mg) orally once daily.
Elderly
No dosage adjustment is generally required, but close monitoring is advised due to potential for renal impairment.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance < 30 mL/min). Dose adjustment may be necessary in moderate impairment.
How to Take
Take the tablet orally once daily, with or without food, at approximately the same time each day.
Mechanism of Action
Telmisartan blocks the action of angiotensin II, a substance in the body that constricts blood vessels, thereby relaxing blood vessels and lowering blood pressure. Hydrochlorothiazide is a diuretic that increases the excretion of sodium and water from the body through urine, which also helps to lower blood pressure.

Telmisartan: Primarily excreted unchanged in feces (97%). Hydrochlorothiazide: Primarily excreted unchanged in urine.

Telmisartan: Approximately 24 hours. Hydrochlorothiazide: 5.6 to 14.8 hours.
Absorption
Telmisartan: Well absorbed, bioavailability 42-58%. Hydrochlorothiazide: Well absorbed, bioavailability 65-75%.
Metabolism
Telmisartan: Conjugated to form an acylglucuronide. Hydrochlorothiazide: Not extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to telmisartan, hydrochlorothiazide, or any sulfonamide-derived drugs.
- Pregnancy (second and third trimesters).
- Severe hepatic or renal impairment (creatinine clearance < 30 mL/min).
- Anuria.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment.
Drug Interactions
NSAIDs
Reduced antihypertensive effect, potential for worsening renal function.
Digoxin
Increased digoxin levels and toxicity due to hydrochlorothiazide-induced hypokalemia.
Lithium
Increased serum lithium concentrations and lithium toxicity.
Other antihypertensives
Additive hypotensive effect.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hypotension, dizziness, tachycardia, bradycardia (due to telmisartan), and electrolyte depletion (due to hydrochlorothiazide). Treatment is symptomatic and supportive. Induce emesis or perform gastric lavage if ingestion is recent. Administer activated charcoal.
Pregnancy & Lactation
Category D for Telmisartan (second and third trimesters), contraindicated during pregnancy due to fetal toxicity. Not recommended during breastfeeding as it may pass into breast milk and potentially harm the infant.
