Description
Overview of the medicine
Telisa 80 mg Tablet contains Telmisartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events like heart attack and stroke in patients at high risk.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. If necessary, the dose may be increased to 80 mg once daily. Maximum recommended dose is 80 mg once daily.
Elderly
No dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment. In severe renal impairment (creatinine clearance <30 mL/min) or patients on hemodialysis, careful monitoring is advised.
How to Take
Telisa tablets should be swallowed whole with water, with or without food, preferably at the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in vascular smooth muscle and the adrenal gland. This blockade prevents the vasoconstricting and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced fluid retention.
Pharmacokinetics
Onset
Antihypertensive effect begins within 3 hours; maximum reduction in blood pressure generally achieved after 4-8 weeks of therapy.
Excretion
Almost exclusively excreted via bile into the feces, predominantly as unchanged drug and its glucuronide conjugate. Renal excretion accounts for less than 1%.
Half life
Approximately 20-24 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is about 42-58%. Peak plasma concentration is reached in 0.5-1 hour.
Metabolism
Metabolized by conjugation to form an acylglucuronide, which is pharmacologically inactive. Minimal metabolism by cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to telmisartan or any component of the formulation.
- Second and third trimesters of pregnancy.
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²).
Drug Interactions
Digoxin
Increased plasma digoxin concentrations when co-administered with telmisartan. Monitor digoxin levels.
Lithium
Concurrent use may increase serum lithium concentrations and lead to lithium toxicity. Monitor lithium levels.
Aliskiren
Increased risk of hyperkalemia, hypotension, and renal impairment when co-administered in patients with diabetes or renal impairment.
Diuretics
May lead to excessive blood pressure reduction.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Telmisartan and increase the risk of renal impairment.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically include symptomatic hypotension and tachycardia; bradycardia might also occur. Treatment is supportive and symptomatic. Hemodialysis is unlikely to be effective in removing telmisartan.
Pregnancy & Lactation
Contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury or death. Not recommended during lactation as it is unknown if Telmisartan is excreted in human milk.

YesClinical Trials
Notable clinical trials involving Telmisartan include ONTARGET (Ongoing Telmisartan Alone and in combination with Ramipril Global Endpoint Trial) which investigated its efficacy in cardiovascular risk reduction, and TRANSCEND (Telmisartan Randomised Assessment Study in ACE intolerant subjects with Cardiovascular Disease) which studied its effects in patients intolerant to ACE inhibitors.
Lab Monitoring
- Regular monitoring of blood pressure.
- Serum potassium levels, especially in patients with renal impairment, heart failure, or those on potassium-sparing diuretics/supplements.
- Renal function (creatinine and GFR) periodically, particularly in elderly patients or those with pre-existing renal dysfunction.
Doctor Notes
- Emphasize the importance of patient adherence to medication, even if asymptomatic.
- Monitor serum potassium and renal function (creatinine, eGFR) regularly, particularly when initiating therapy or titrating dose.
- Counsel patients on potential for orthostatic hypotension, especially in volume-depleted or elderly patients.
- Strictly contraindicated in 2nd and 3rd trimesters of pregnancy; ensure adequate contraception for women of childbearing potential.
Patient Guidelines
- Take Telisa exactly as prescribed by your doctor.
- Do not stop taking Telisa without consulting your doctor, even if you feel well.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
- Report any swelling of the face, lips, tongue, or throat (angioedema) immediately to your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Telisa may cause dizziness or lightheadedness, especially at the start of treatment or when changing dose. If affected, avoid driving or operating machinery.
Lifestyle Advice
- Adopt a healthy lifestyle including a balanced diet, regular exercise, and maintaining a healthy weight.
- Limit sodium intake, avoid smoking, and reduce alcohol consumption.
