Description
Overview of the medicine
Telmivas is a combination medication used to treat high blood pressure (hypertension). It contains Telmisartan, an Angiotensin II Receptor Blocker (ARB), and Amlodipine, a Calcium Channel Blocker (CCB). These two medications work together to relax blood vessels, allowing blood to flow more easily and thereby lowering blood pressure.
Uses & Indications
Dosage
Adults
Typically, one Telmivas 40 mg + 5 mg tablet once daily. Dosage may be adjusted based on individual response and tolerability, up to a maximum of 80 mg + 10 mg daily.
Elderly
No specific dose adjustment is generally required, but caution should be exercised due to potential age-related decrease in renal function.
Renal_impairment
No dose adjustment is necessary for mild to moderate renal impairment. For severe impairment, closer monitoring is recommended.
How to Take
Take orally, with or without food, at the same time each day.
Mechanism of Action
Telmisartan blocks the action of angiotensin II, a substance in the body that constricts blood vessels, leading to lower blood pressure. Amlodipine relaxes blood vessels and improves blood flow by blocking calcium from entering the muscle cells of the heart and blood vessels.

Telmisartan: gradual, full effect in 4-8 weeks. Amlodipine: 6-12 hours.
Excretion
Telmisartan: mainly excreted unchanged in feces. Amlodipine: primarily excreted via urine as inactive metabolites.

Telmisartan: approximately 24 hours. Amlodipine: 30-50 hours.
Absorption
Telmisartan is well absorbed, about 42-58%, with peak plasma concentrations occurring within 0.5-1 hour. Amlodipine is well absorbed, with peak concentrations reached in 6-12 hours, and bioavailability of 64-90%.
Metabolism
Telmisartan: minimally metabolized, mainly by glucuronidation. Amlodipine: extensively metabolized in the liver to inactive metabolites.

Contraindications
- Hypersensitivity to telmisartan, amlodipine, or any component of the formulation

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include marked hypotension and tachycardia (or bradycardia). Treatment is symptomatic and supportive. Amlodipine overdose may require gastric lavage and activated charcoal. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Contraindicated in the 2nd and 3rd trimesters of pregnancy. Advise women of childbearing potential about the risk of exposure. Not recommended during breastfeeding due to potential risk to the infant.
