Overview of the medicine
Telsan 40 mg Tablet contains Telmisartan, an Angiotensin II Receptor Blocker (ARB), used to treat high blood pressure (hypertension) and reduce the risk of cardiovascular events such as heart attack or stroke.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily. The dose may be increased to a maximum of 80 mg once daily if necessary to achieve blood pressure control.
Elderly
No dosage adjustment is generally required for elderly patients. Close monitoring is advised.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment or hemodialysis, initial dose should be 20 mg once daily.
How to Take
Telsan 40 mg tablet should be taken orally once daily with or without food. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, leading to vasodilation, reduced aldosterone secretion, and overall reduction in blood pressure. It does not inhibit ACE.

Within 3 hours for antihypertensive effect.
Excretion
Primarily excreted unchanged via bile into feces (over 97%). Minimal renal excretion.
Half life
Approximately 24 hours, allowing for once-daily dosing.
Absorption
Rapidly but variably absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 42-58%. Food slightly reduces absorption.
Metabolism
Almost exclusively metabolized by conjugation to form the pharmacologically inactive acylglucuronide. Not metabolized by cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Second and third trimesters of pregnancy.
- Biliary obstructive disorders.
- Severe hepatic impairment.
- Concomitant use of aliskiren in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m²).
Drug Interactions
Digoxin
Increased plasma concentrations of digoxin. Monitor digoxin levels.
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels.
Aliskiren
Contraindicated in patients with diabetes or renal impairment.
NSAIDs (e.g., ibuprofen, naproxen)
Reduced antihypertensive effect and increased risk of worsening renal function.
Diuretics (e.g., furosemide, hydrochlorothiazide)
Increased risk of hypotension and renal impairment.
Potassium-sparing diuretics (e.g., spironolactone) or potassium supplements
Increased risk of hyperkalemia.

Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Limited data are available on overdose. The most likely manifestations would be hypotension and tachycardia; bradycardia could also occur. Treatment is symptomatic and supportive. Telmisartan is not removed by hemodialysis.
Pregnancy & Lactation
Telsan is contraindicated during the second and third trimesters of pregnancy due to risks of fetal injury and death. It is not recommended during the first trimester. Discontinue as soon as pregnancy is detected. Telmisartan is also contraindicated during lactation due to potential adverse effects on the nursing infant.
