Description
Overview of the medicine
Temfin 500 mg tablet contains Paracetamol, which is an effective medicine for relieving mild to moderate pain and reducing fever. It works by blocking pain signals and reducing heat in the body.
Uses & Indications
Dosage
Adults
1-2 tablets (500-1000 mg) every 4-6 hours as needed. Maximum 8 tablets (4000 mg) in 24 hours.
Elderly
No specific dose reduction is typically required for elderly patients, but caution is advised in those with impaired renal or hepatic function. Follow adult dosage.
Renal_impairment
For creatinine clearance 10-50 ml/min: 500 mg every 6 hours. For creatinine clearance <10 ml/min: 500 mg every 8 hours.
How to Take
Take the tablet orally with water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Paracetamol (Acetaminophen) is believed to act predominantly in the central nervous system, increasing the pain threshold by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are involved in prostaglandin synthesis. Its antipyretic effect is achieved through action on the hypothalamic heat-regulating center.
Pharmacokinetics
Onset
30-60 minutes for analgesic and antipyretic effects.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1.25 to 3 hours in adults.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by cytochrome P450 enzymes to a toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is rapidly detoxified by conjugation with glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation.
- Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Concomitant use of alcohol can increase the risk of hepatotoxicity.
Warfarin
May increase the anticoagulant effect of warfarin, leading to an increased risk of bleeding, especially with chronic high doses.
Cholestyramine
Reduces paracetamol absorption if administered within one hour.
Certain anti-epileptics (e.g., Phenytoin, Carbamazepine)
May increase the metabolism of paracetamol, reducing its effectiveness and potentially increasing the risk of liver damage.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose of paracetamol can cause severe, potentially fatal liver damage. Symptoms may include nausea, vomiting, abdominal pain, and sweating, appearing 24-48 hours after ingestion. Treatment involves gastric lavage, activated charcoal (if within one hour of ingestion), and administration of N-acetylcysteine as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at recommended doses. However, always consult a doctor before taking any medication during these periods.

Lab Monitoring
- Liver function tests (ALT, AST) are recommended in cases of suspected overdose or prolonged high-dose therapy.
- Renal function tests may be considered in chronic overuse.
Doctor Notes
- Emphasize strict adherence to dosing limits (max 4g/day for adults) to prevent hepatotoxicity.
- Advise patients to check other medications for paracetamol content to avoid accidental overdose.
- Counsel on early symptoms of liver damage (nausea, vomiting, abdominal pain) in case of overdose suspicion.
- Caution in patients with pre-existing liver disease or chronic alcohol use.
Patient Guidelines
- Do not exceed the recommended dose of Temfin.
- Avoid taking other products containing paracetamol simultaneously.
- Consult a doctor if pain or fever persists for more than a few days.
- Do not take for more than 3 days for fever or 10 days for pain without consulting a doctor.
