Description
Overview of the medicine
Tempac 120 mg Suspension is used for the symptomatic relief of mild to moderate pain and fever. It contains Paracetamol (Acetaminophen) as its active ingredient.
Uses & Indications
Dosage
Adults
Not typically prescribed as 120 mg suspension for adults. For other formulations, 500-1000 mg every 4-6 hours, maximum 4000 mg/day.
Children
10-15 mg/kg body weight per dose, every 4-6 hours as needed. Do not exceed 5 doses in 24 hours. Refer to specific age/weight guidelines on product packaging or consult a doctor. For example, 1-5 years: 5-10 ml (120-240 mg).
Renal_impairment
Dose adjustment may be necessary in patients with severe renal impairment (creatinine clearance < 10 mL/min). Interval between doses should be extended.
How to Take
For oral administration. Shake the bottle well before use. Can be taken with or without food. Use the provided measuring device (spoon or dropper) to ensure accurate dosing.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system (CNS) by inhibiting prostaglandin synthesis, which reduces pain and fever. Its peripheral anti-inflammatory activity is minimal.
Pharmacokinetics
Onset
Analgesic effect begins within 30-60 minutes, antipyretic effect within 30 minutes.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma half-life is typically 1.25 to 3 hours; may be prolonged in liver disease or overdose.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30-60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by cytochrome P450 to a toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is normally detoxified by glutathione.
Side Effects
Contraindications
- Hypersensitivity to Paracetamol or any other components of the formulation
- Severe hepatic impairment or active liver disease
Drug Interactions
Cholestyramine
May decrease the absorption of Paracetamol if taken within one hour.
Warfarin and other coumarins
May enhance the anticoagulant effect, leading to increased bleeding risk, especially with prolonged use of high doses of Paracetamol.
Metoclopramide and Domperidone
May increase the absorption rate of Paracetamol.
Carbamazepine, Phenobarbital, Phenytoin, Rifampicin
May increase the risk of hepatotoxicity due to induction of hepatic enzymes.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Do not freeze. Keep the bottle tightly closed.
Overdose
Overdose of Paracetamol can cause severe, potentially fatal, liver damage. Symptoms may include nausea, vomiting, abdominal pain, and sweating, appearing 24-48 hours after ingestion. Treatment involves immediate medical attention, gastric lavage, activated charcoal (if within one hour of ingestion), and administration of the antidote N-acetylcysteine.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at therapeutic doses. However, it should be used with caution and only if clearly needed, under medical supervision.

YesClinical Trials
Paracetamol is one of the most extensively studied and widely used medications globally. Numerous clinical trials have confirmed its efficacy and safety for fever and pain relief, particularly in pediatric and general adult populations when used at recommended doses.
Lab Monitoring
- Liver function tests (ALT, AST) are recommended for patients on long-term or high-dose Paracetamol therapy, or in cases of suspected overdose. Renal function may also be monitored.
Doctor Notes
- Emphasize to parents the importance of accurate dosing based on weight/age and avoiding concomitant paracetamol-containing products to prevent overdose.
- Advise on monitoring for signs of liver toxicity in cases of chronic high-dose use or suspected overdose.
- Educate patients on the safe use of OTC analgesics.
Patient Guidelines
- Always use the provided measuring device for accurate dosing, especially for children.
- Do not exceed the recommended dose or frequency of administration.
- Consult a doctor if fever or pain persists for more than 3-5 days.
- Keep out of reach of children.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Paracetamol is generally not expected to affect the ability to drive or operate machinery. However, if you experience dizziness or other side effects, avoid such activities.
Lifestyle Advice
- Ensure adequate fluid intake when you have a fever.
- Rest is important for recovery from fever or illness.
- Avoid alcohol while taking paracetamol, especially if you have liver conditions.
