Overview of the medicine
Tempanil 500 mg Tablet is a medication containing Paracetamol, primarily used for relieving mild to moderate pain and reducing fever. It works by blocking chemical messengers in the brain that tell us we have pain and are hot.
Uses & Indications
Dosage
Adults
Oral: 500 mg to 1000 mg (1-2 tablets) every 4 to 6 hours as needed. Do not exceed 4000 mg (8 tablets) in 24 hours.
Elderly
Similar to adult dosage; however, caution should be exercised in patients with impaired renal or hepatic function.
Renal_impairment
Dosage interval may need to be extended in severe renal impairment (e.g., 500 mg every 6-8 hours for CrCl < 10 mL/min).
How to Take
Tempanil 500 mg Tablet should be taken orally with water. It can be taken with or without food. Do not chew or crush the tablet.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system (CNS) by inhibiting prostaglandin synthesis, which reduces pain and fever. It has weak peripheral anti-inflammatory activity.

Excretion
Excreted primarily in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.

Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30 to 60 minutes after oral administration.
Metabolism
Extensively metabolized in the liver mainly by conjugation with glucuronic acid and sulfate. A small portion is metabolized by cytochrome P450 enzymes to a toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is rapidly detoxified by conjugation with glutathione.

Drug Interactions
Alcohol
Increased risk of hepatotoxicity when consumed chronically and excessively with paracetamol.
Cholestyramine
May decrease the absorption of paracetamol if taken within one hour.
Metoclopramide and Domperidone
May increase the absorption rate of paracetamol.
Warfarin and other coumarin anticoagulants
Increased anticoagulant effect, leading to increased bleeding risk, particularly with prolonged use of high doses of paracetamol.
