Description
Overview of the medicine
Tempol-Plus 500 mg Tablet is a combination medication containing Paracetamol and Orphenadrine Citrate. It is used for the relief of mild to moderate pain, especially when associated with skeletal muscle spasm.
Uses & Indications
Dosage
Adults
1-2 tablets three times daily, or as directed by the physician.
Elderly
Lower doses may be required due to increased sensitivity to anticholinergic effects and potential for impaired renal/hepatic function.
Renal_impairment
Use with caution. Dosage adjustment may be necessary in severe impairment. Consult a physician.
How to Take
Take the tablet orally with water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Paracetamol exerts its analgesic and antipyretic effects by inhibiting prostaglandin synthesis, primarily in the central nervous system. Orphenadrine Citrate is a centrally acting skeletal muscle relaxant that works through its anticholinergic properties and by reducing nerve impulses from the brain to the skeletal muscles.
Pharmacokinetics
Onset
Paracetamol: 30-60 minutes; Orphenadrine: Approximately 1 hour.
Excretion
Mainly excreted in the urine, primarily as glucuronide and sulfate conjugates for Paracetamol. Orphenadrine and its metabolites are also excreted renally.
Half life
Paracetamol: 1-3 hours; Orphenadrine: 14-16 hours.
Absorption
Paracetamol is rapidly and well absorbed from the GI tract. Orphenadrine is also readily absorbed.
Metabolism
Both Paracetamol and Orphenadrine are extensively metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Paracetamol, Orphenadrine, or any component of the formulation
- Glaucoma
- Prostatic hypertrophy or bladder neck obstruction
- Myasthenia gravis
- Pyloric or duodenal obstruction
- Esophageal spasm
Drug Interactions
Alcohol
Concurrent use of alcohol with Paracetamol increases the risk of hepatotoxicity. Orphenadrine may enhance the sedative effects of alcohol.
Warfarin
Paracetamol may enhance the anticoagulant effect of warfarin, increasing bleeding risk.
Anticholinergic drugs
Orphenadrine's anticholinergic effects may be enhanced when used with other anticholinergic agents (e.g., tricyclic antidepressants, phenothiazines).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose of Paracetamol can cause severe liver damage, sometimes fatal. Overdose of Orphenadrine can lead to anticholinergic effects like severe dry mouth, blurred vision, rapid heartbeat, confusion, and hallucinations. Seek immediate medical attention in case of overdose.
Pregnancy & Lactation
Paracetamol is generally considered safe for use in pregnancy at recommended doses. Orphenadrine Citrate should be used with caution during pregnancy and lactation only if clearly needed and the potential benefits outweigh the risks. Consult a physician.
