Description
Overview of the medicine
Teneligliptin is an oral anti-diabetic drug indicated for the treatment of type 2 diabetes mellitus. It belongs to the class of dipeptidyl peptidase-4 (DPP-4) inhibitors, which work by enhancing the body's natural ability to lower high blood glucose levels.
Uses & Indications
Dosage
Adults
The usual starting dose of Teneligliptin is 20 mg once daily. For a 3 mg tablet, the dosage should be precisely determined by a healthcare professional based on individual glycemic control, patient needs, and tolerability. Do not self-medicate or adjust dose.
Elderly
No specific dose adjustment is generally required for elderly patients based on age alone. However, renal and hepatic function should be monitored, and dosage adjusted if necessary.
Renal_impairment
No dose adjustment is generally required for Teneligliptin in patients with renal impairment, but caution is advised in severe cases. For a 3 mg tablet, consult a physician.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Teneligliptin selectively inhibits the enzyme dipeptidyl peptidase-4 (DPP-4), which is responsible for inactivating incretin hormones (GLP-1 and GIP). By inhibiting DPP-4, Teneligliptin increases the levels of active incretin hormones, leading to enhanced glucose-dependent insulin secretion from pancreatic beta cells and reduced glucagon secretion from pancreatic alpha cells. This helps to improve glycemic control.
Pharmacokinetics
Onset
Glycemic control effects observed within hours after first dose.
Excretion
Excreted through both renal (kidneys) and hepatobiliary (liver and bile) pathways, with roughly equal proportions.
Half life
Approximately 24 hours, supporting once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentration (Tmax) usually achieved within 1-2 hours after oral administration. High bioavailability.
Metabolism
Primarily metabolized by cytochrome P450 enzymes (CYP3A4 and CYP2D6) and flavin-containing monooxygenase (FMO1, FMO3) in the liver.
Side Effects
Contraindications
- Known hypersensitivity to Teneligliptin or any excipients of the tablet.
- Patients with type 1 diabetes mellitus.
- Patients with diabetic ketoacidosis.
Drug Interactions
Digoxin
Monitor digoxin levels if co-administered, as Teneligliptin may slightly increase digoxin exposure.
Strong CYP3A4/2D6 Inhibitors/Inducers
Caution is advised; however, significant clinical interactions are generally not expected due to balanced metabolism pathways.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated as appropriate. Teneligliptin is not significantly removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Teneligliptin is excreted in human milk, so caution should be exercised when administered to a nursing mother. Consult your doctor.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of Teneligliptin in improving glycemic control in patients with type 2 diabetes mellitus, both as monotherapy and in combination with other agents. Ongoing studies continue to explore its long-term cardiovascular outcomes and broader applications.
Lab Monitoring
- Blood glucose levels (fasting and post-prandial)
- HbA1c (every 3-6 months)
- Renal function (e.g., creatinine, eGFR)
- Liver function tests (e.g., ALT, AST) intermittently
Doctor Notes
- Emphasize the importance of patient education regarding diet, exercise, and adherence to medication.
- Monitor for signs and symptoms of pancreatitis and bullous pemphigoid.
- Advise patients about the risk of hypoglycemia, especially when Teneligliptin is co-administered with sulfonylureas or insulin.
- Regularly assess renal and hepatic function, although dose adjustment for Teneligliptin is usually not required for mild to moderate impairment.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking Tenapam-3-mg-tablet without consulting your doctor, even if you feel well.
- Maintain a healthy diet and regular exercise alongside medication for optimal results.
- Be aware of symptoms of hypoglycemia, especially if taking with other anti-diabetic medications.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Teneligliptin generally has no or negligible influence on the ability to drive and use machines. However, patients should be advised of the risk of hypoglycemia when Teneligliptin is used in combination with sulfonylureas or insulin, which may impair driving ability.
Lifestyle Advice
- Follow a balanced diet low in sugar and saturated fats.
- Engage in regular physical activity for at least 30 minutes most days of the week.
- Maintain a healthy weight.
- Monitor blood glucose levels regularly as advised by your doctor.
