Description
Overview of the medicine
Teneligliptin is an oral anti-diabetic drug used to improve glycemic control in adults with type 2 diabetes mellitus. It belongs to the dipeptidyl peptidase-4 (DPP-4) inhibitor class.
Uses & Indications
Dosage
Adults
20 mg (four 5 mg tablets) once daily. May be increased to 40 mg (eight 5 mg tablets) once daily if needed, based on glycemic control.
Elderly
No dose adjustment generally required based on age alone. Caution is advised for elderly patients with renal impairment.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. For severe renal impairment or end-stage renal disease, caution and close monitoring are advised.
How to Take
Orally, once daily, with or without food.
Mechanism of Action
Teneligliptin inhibits dipeptidyl peptidase-4 (DPP-4) enzyme, which is responsible for inactivating incretin hormones (GLP-1 and GIP). By inhibiting DPP-4, Teneligliptin increases the levels of active incretin hormones, leading to increased insulin secretion and decreased glucagon secretion in a glucose-dependent manner, thereby lowering blood glucose levels.

Onset
Within hours of administration, sustained glucose-lowering effect.
Excretion
Excreted renally and hepatically. Approximately 60-70% via urine and 20-30% via feces.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed, peak plasma concentration reached within 1-2 hours. Bioavailability is high (approx. 100%).
Metabolism
Primarily metabolized by CYP3A4 and flavin-containing monooxygenase 3 (FMO3).
Side Effects
Contraindications
- •Hypersensitivity to Teneligliptin or any component of the tablet
- •Type 1 Diabetes Mellitus
- •Diabetic ketoacidosis
Drug Interactions
Digoxin
May increase digoxin concentration slightly, monitor digoxin levels.
CYP3A4/FMO3 inhibitors (e.g., ketoconazole)
May increase Teneligliptin exposure, though clinically significant interactions are rare.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to hypoglycemia. Management includes symptomatic and supportive measures. Gastric lavage and activated charcoal may be considered in acute overdose.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excretion into breast milk is unknown, use with caution.
