Description
Overview of the medicine
Tenizide 20 mg tablet is a thiazide-like diuretic used to treat high blood pressure (hypertension) and edema caused by various conditions such as heart failure, kidney disease, or liver disease. It helps the body get rid of excess water and salt.
Uses & Indications
Dosage
Adults
For hypertension: Initially 10-20 mg once daily. May be increased to a maximum of 40 mg daily if needed. For edema: 20-40 mg once daily, or on alternate days, depending on severity.
Elderly
Lower initial doses (e.g., 10 mg once daily) may be appropriate, with careful titration, due to potential for reduced renal function and increased sensitivity.
Renal_impairment
Use with caution. Not recommended in severe renal impairment (creatinine clearance <30 mL/min). Dosage adjustment may be necessary in moderate impairment.
How to Take
Take Tenizide tablet orally, usually once daily, with or without food. It is recommended to take it in the morning to avoid nocturnal urination. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Tenizide primarily works by inhibiting the sodium-chloride symporter in the distal convoluted tubule of the kidney. This action prevents the reabsorption of sodium and chloride ions, leading to increased excretion of sodium, chloride, potassium, and water in the urine, thereby reducing blood volume and peripheral vascular resistance.
Pharmacokinetics
Onset
Diuretic effect usually begins within 2 hours.
Excretion
Primarily excreted unchanged by the kidneys (approximately 65-70% within 24 hours).
Half life
Elimination half-life ranges from 6 to 15 hours, depending on renal function.
Absorption
Well absorbed from the gastrointestinal tract, with bioavailability typically around 60-70%.
Metabolism
Undergoes minimal hepatic metabolism.
Side Effects
Contraindications
- Hypersensitivity to Tenizide or other sulfonamide-derived drugs
- Anuria (absence of urine production)
- Severe renal impairment (creatinine clearance <30 mL/min)
- Severe hepatic impairment or hepatic coma
Drug Interactions
Lithium
May reduce renal clearance of lithium, leading to increased serum lithium levels and potential toxicity.
Corticosteroids
May enhance electrolyte depletion, particularly hypokalemia.
Antidiabetic Agents
May decrease the glucose-lowering effect of antidiabetic drugs; dosage adjustment may be required.
Digitalis Glycosides
Hypokalemia induced by Tenizide may enhance the cardiac effects and toxicity of digitalis.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the diuretic, natriuretic, and antihypertensive effects of Tenizide.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include electrolyte depletion (hypokalemia, hyponatremia, hypochloremia), dehydration, hypotension, and dizziness. Management involves symptomatic and supportive measures, including correction of fluid and electrolyte imbalances. Gastric lavage or activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Tenizide is excreted in human milk; caution should be exercised when administered to a nursing mother. Consult your doctor before use during pregnancy or lactation.

Lab Monitoring
- Serum electrolytes (potassium, sodium, chloride) regularly, especially during initial therapy and dosage changes.
- Renal function (creatinine, BUN) periodically.
- Blood glucose levels (especially in diabetic patients).
- Uric acid levels.
Doctor Notes
- Advise patients to monitor for symptoms of electrolyte imbalance (e.g., muscle weakness, palpitations).
- Consider potassium supplementation or potassium-sparing diuretics if hypokalemia is significant.
- Educate patients on the importance of a low-sodium diet.
Patient Guidelines
- Take Tenizide exactly as prescribed by your doctor.
- Do not stop taking this medicine without consulting your doctor, even if you feel well.
- Report any unusual symptoms or side effects to your doctor immediately.
- Avoid excessive sun exposure as it may increase sensitivity.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Tenizide may cause dizziness or lightheadedness, especially at the beginning of treatment or when changing doses. Exercise caution when driving or operating machinery until you know how the medicine affects you.
Lifestyle Advice
- Maintain a low-sodium diet to enhance the blood pressure-lowering effect.
- Engage in regular physical activity as advised by your doctor.
- Limit alcohol consumption.
- Monitor your blood pressure and weight regularly.
