Overview of the medicine
Tenobis-A 25 mg tablet is a combination medication containing Teneligliptin, an antidiabetic, and Amlodipine, an antihypertensive. It is used to treat patients with type 2 diabetes mellitus and hypertension when treatment with both active ingredients is appropriate.
Uses & Indications
Dosage
Adults
One tablet once daily. Dosage may be individualized based on the patient's current glycemic control, blood pressure, and tolerance to the individual components.
Elderly
No specific dose adjustment is generally required, but caution should be exercised due to potential age-related decline in renal and hepatic function.
Renal_impairment
For Teneligliptin, no dose adjustment is needed for mild to moderate renal impairment; for severe impairment, close monitoring. For Amlodipine, no dose adjustment is required for renal impairment. Consult a physician.
How to Take
Take orally, once daily, with or without food, at approximately the same time each day.
Mechanism of Action
Teneligliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that prevents the breakdown of incretins, leading to increased insulin secretion and reduced glucagon levels, thus lowering blood glucose. Amlodipine is a dihydropyridine calcium channel blocker that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, causing peripheral vasodilation and reduction in blood pressure.

Teneligliptin: Blood glucose lowering effects within hours. Amlodipine: Antihypertensive effects typically begin within 6-9 hours.

Teneligliptin: Primarily via urine and feces. Amlodipine: Excreted mainly in the urine as inactive metabolites, with less than 10% as unchanged drug.

Teneligliptin: Approximately 24 hours (effective half-life). Amlodipine: 30-50 hours.
Absorption
Teneligliptin is rapidly absorbed with good bioavailability. Amlodipine is slowly but almost completely absorbed from the GI tract (bioavailability 64-90%).
Metabolism
Teneligliptin: Primarily metabolized by CYP3A4 and FMO3. Amlodipine: Extensively metabolized in the liver to inactive metabolites via CYP3A4.

Contraindications
- Hypersensitivity to Teneligliptin, Amlodipine, or any component of the formulation.
- Diabetic ketoacidosis.
- Acute pancreatitis or history of pancreatitis (for Teneligliptin).
- Cardiogenic shock, unstable angina, severe aortic stenosis (for Amlodipine).
Drug Interactions
Digoxin
Teneligliptin may cause a minor increase in Digoxin levels.
CYP3A4 Inducers (e.g., Rifampicin)
May decrease Amlodipine plasma concentrations.
CYP3A4 Inhibitors (e.g., Ketoconazole, Diltiazem)
