Description
Overview of the medicine
Tenoxim 20 mg Tablet contains Piroxicam, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by arthritis and other inflammatory conditions. It works by blocking the body's production of certain natural substances that cause inflammation.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily, or 10 mg twice daily, for inflammatory conditions. For acute musculoskeletal disorders, 40 mg once daily for the first two days, then 20 mg once daily for 7 to 14 days.
Elderly
Use with caution; lower doses may be considered due to increased risk of adverse effects. Generally, the lowest effective dose for the shortest duration.
Renal_impairment
Reduced dosage may be necessary; monitor renal function closely. Not recommended for severe renal impairment.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Piroxicam is a non-selective NSAID that inhibits the activity of cyclooxygenase (COX-1 and COX-2) enzymes. This inhibition reduces the synthesis of prostaglandins, which are mediators of inflammation, pain, and fever.
Pharmacokinetics
Onset
Analgesic effect usually starts within 1 hour, anti-inflammatory effects may take several days to weeks.
Excretion
Excreted predominantly in the urine (approximately 60%) and feces (approximately 40%) as metabolites.
Half life
Approximately 30-86 hours (average 50 hours), allowing for once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract, reaching peak plasma concentrations typically within 3-5 hours after oral administration.
Metabolism
Extensively metabolized in the liver, primarily via hydroxylation, followed by conjugation with glucuronic acid.

Increased plasma lithium levels, leading to toxicity.
Methotrexate
Increased plasma methotrexate levels, leading to toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects, increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Antiplatelet agents (e.g., Aspirin, Clopidogrel)
Increased risk of gastrointestinal bleeding.
ACE inhibitors/Angiotensin Receptor Blockers (ARBs)
Reduced antihypertensive effect, increased risk of renal impairment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, lethargy, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves symptomatic and supportive care. Gastric lavage and activated charcoal may be considered if recent ingestion. Monitor renal function and acid-base balance.
Pregnancy & Lactation
